摘要
目的探讨血清糖类抗原125(CA125)、β-人绒毛膜促性腺激素(β-h CG)和血管内皮生长因子(VEGF)在早期异位妊娠中的表达变化,对三指标联检在异位妊娠中早期诊断及药物治疗预后中的价值进行研究。方法以该院2015年1月-2017年2月期间收治的异位妊娠患者82例为研究对象,设为研究组,并根据药物治疗是否有效分为有效组和无效组两个亚组。选取同期在该院行常规体检的正常妊娠孕妇80例为对照组,应用ELISA法测定两组血清CA125、β-h CG、VEGF水平。分析研究组治疗前、后3个指标水平的变化,探讨其临床价值。结果研究组血清β-h CG水平低于对照组,而CA125、VEGF水平明显高于对照组(P<0.05);研究组的有效组β-h CG高于无效组,CA125、VEGF表达低于无效组(均P<0.05);三指标联检时ROC曲线下面积、阴性预测值、阳性预测值、灵敏性、特异性均高于各单项指标检测结果。结论血清CA125、β-h CG与VEGF是异位妊娠早期诊断及药物保守治疗预后的有效评价指标,而通过3种治疗联检可提高异位妊娠临床诊断准确性。
Objective To explore the changes of expression levels of serum carbohydrate antigen 125（ CA125）,β-human chorionic gonadotropin（ β-hCG）,and vascular endothelial growth factor（ VEGF） in early ectopic pregnancy,research the value of combined detection of serum CA125,β-hCG,and VEGF in early diagnosis of ectopic pregnancy and prognosis of drug therapy. Methods From January2015 to February 2017,82 patients with ectopic pregnancy treated in Shanxi Provincial Coal Central Hospital were selected as study group,then the patients were further divided into effective subgroup and ineffective subgroup. Eighty normal pregnant women receiving general examination in the hospital during the same period were selected as control group. ELISA was used to detect the levels of serum CA125,β-hCG,and VEGF in the two groups. The changes of CA125,β-hCG,and VEGF levels before and after treatment in study group were analyzed,and the clinical value was discussed. Results The level of serum β-hCG in study group was lower than that in control group,while the levels of serum CA125 and VEGF in study group were higher than those in control group,there were statistically significant differences（ P 0. 05）; the level of β-hCG in effective subgroup was higher than that in ineffective group,while the levels of serum CA125 and VEGF in effective subgroup were lower than those in ineffective group,there were statistically significant differences（ P〈0. 05）. The levels of area under ROC curve,negative predictive value,positive predictive value,sensitivity,and specificity of combined detection of serum CA125,β-hCG,and VEGF were higher than those of single detection of CA125,β-hCG,and VEGF. Conclusion Serum CA125,β-hCG,and VEGF are effective evaluation indexes of early diagnosis of ectopic pregnancy and prognosis of drug therapy,combined detection of serum CA125,β-hCG,and VEGF can improve the clinical accuracy of diagnosis of ectopic pregnancy.
