摘要
目的探讨肝移植患者血清肝功能指标在术后恢复过程中的规律和特点。方法回顾性分析88例肝移植患者术前1 d和术后1 d、3 d、7 d、30 d及1年血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、谷氨酰转肽酶(GGT)、碱性磷酸酯酶(ALP)、总胆红素(TBIL)、直接胆红素(DBIL)、总胆汁酸(TBA)的变化情况。结果 ALT术后1 d急剧升高,3 d开始大幅下降,1年后降至术前水平。AST术后1 d急剧升高,3 d迅速回落,7 d降至低于术前水平。GGT术后1 d降低,3 d恢复到术前水平,而后又升高,1年后降低至术前水平。ALP术后1 d开始降低,直至7 d均维持在较低水平,30 d恢复至术前水平,术后1年保持与术前相当水平。TBIL、DBIL及TBA术后1 d开始逐渐下降,30 d接近正常水平,1年后保持该水平。结论肝移植患者血清肝功能指标的变化具有一定的规律性,对于指导临床诊断和治疗有一定价值。
Objective To explore the changes and characteristics of serum liver function indexes in patients after liver transplantation.Methods The levels of serum alanine aminotransferase（ALT）,aspartate aminotransferase（AST）,glutamyl transpeptidase（GGT）,alkaline phosphatase（ALP）,total bilirubin（TBIL）,direct bilirubin（DBIL）and total bile acid（TBA）of 88 patients who underwent liver transplantation were measured at 1 d before operation,and 1 d,3 d,7 d,30 d and 1 year after operation.Results ALT increased sharply at 1 d after operation,decreased significantly at 3 d,and then decreased to the preoperative level 1 year after operation.AST increased rapidly at 1 d after operation,then declined at 3 d,fell to a level lower than preoperative level at 7 d after operation.GGT decreased at 1 d and rebound to preoperative level at 3 d,and then went up,and finally fell to preoperative level 1 year after operation.ALP decreased from 1 d to 7 d and rebound to preoperative level at30 d and 1 year after operation.The levels of TBIL,DBIL and TBA decreased gradually from 1 d to 30 d,and remained at the normal level 1 year after operation.Conclusion There is a certain regularity in the changes of serum liver function indexes in patients who underwent liver transplantation,which may be valuable for guiding clinical diagnosis and treatment.
