摘要
目的探讨99mTc-热变性红细胞脾显像,对脾破裂脾脏切除术后异位脾种植的诊断价值。方法 (1)99mTc标记热变性红细胞(改良方法);(2)Philips Forte双探头SPECT;(3)进行早期和延迟显像;(4)复习国内文献。结果 (1)患者外伤后脾切除、肠穿孔修补术22年。99mTc-热变性红细胞脾显像发现异位脾种植6枚,其中5枚通过增强CT定位,1枚CT平扫定位于子宫表面;(2)复习文献9篇,临床以肿瘤收治61例,手术61例(其中5例穿刺活检,3例腔镜活检),诊断脾异位种植超过68个。结论99mTc-热变性红细胞脾显像特异性诊断异位脾种植可避免不必要手术,保留患者残存防御能力,值得临床推广应用。
Objective To discuss diagnostic value of ^99mTc-thermal denaturation red blood cell spleen imaging for ectopic spleen implantation after splenectomy for splenic rupture. Methods (1) ^99mTc-thermal denaturation red blood cell(modified method) ;(2)Philips Forte double probe SPECT; (3)earlier and delayed imaging; (4)reviewing domestic literatures. Results (1)The patient were underwent traumatic splenectomy and intestinal perforation for 22 years. 6 ectopic spleen implantations were found by ^99mTc-thermal denaturation red blood cell spleen imaging,in which five were located through enhanced CT,and the rest one was located on surface of uterus by CT plain scan. (2)Reviewing 9 literatures, 61 cases were adopted as neoplasm, surgical operations in 61 including 5 puncture biospy and 3 laparoscopy biospy, and more than 68 ectopic spleen implantations diagnosed. Conclusion Specific diagnosis of ectopic spleen implantations by ^99mTcTc-thermal denaturation red blood cell spleen imaging can avoid unnecessary operations and remain patients' remnant defense abilities. It has clinical value and worth spreading application.
