联合检测IFN-γ、TNF-α和IL-2对结核性胸腔积液和恶性胸腔积液鉴别诊断的价值 被引量:13

摘要 本研究通过检测外周血和胸腔积液中γ-干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)和白细胞介素-2(IL-2)的水平变化,从而探究这3种细胞因子对结核性胸腔积液和恶性胸腔积液的鉴别诊断的临床价值。本研究选取2014年12月至2016年10月我院收治的共78例胸腔积液患者,其中结核性胸腔积液组40例、恶性胸腔积液组38例,本实验采用ELISA法分别检测这两组患者外周血和胸水中IFN-γ、TNF-α和IL-2的水平。根据实验数据结果分析两组患者外周血和胸水中IFN-γ、TNF-α和IL-2水平的差异。结果显示,结核性胸腔积液组外周血和胸腔积液中IFN-γ、TNF-α和IL-2的水平均明显高于恶性胸腔积液组(p〈0.05);通过ROC曲线可知诊断结核性胸膜炎的灵敏度分别为90.3%、91.2%和97.5%,特异度分别为79.5%、89.3%和86.6%。在外周血和胸腔积液检测IFN-γ、TNF-α和IL-2的水平对结核性和恶性胸腔积液性质的鉴别诊断具有重要临床参考价值。 In this study,the levels of IFN-γ,TNF-αand IL-2 in peripheral blood and pleural effusion were detectedto explore the clinical value of three cytokines in the differential diagnosis of tuberculous pleural effusion and malignant pleural effusion.This study selected a total of 78 cases of pleural effusion patients treated in our hospital from December 2014 to October 2016,including 40 cases of tuberculous pleural effusion group,38 cases of malignant pleural effusion group.The levels of IFN-γ,TNF-αand IL-2 in peripheral blood and pleural effusion of two groups of patientswere deteced by ELISA.According to the experimental data,the differences of the levels of IFN-γ,TNF-αand IL-2 in peripheral blood and pleural effusion between the two groups were analyzed.The results showed that the levels of IFN-γ,TNF-αand IL-2 in peripheral blood and pleural effusion of tuberculous pleural effusion group were significantly higher than those of malignant pleural effusion group(p〈0.05).Acco rding to the ROC curve,sensitivities of tuberculous pleural effusion were 90.3%,91.2%and 97.5%respectively,with specificities of 79.5%,89.3%and 86.6%,respectively.The levels of IFN-γ,TNF-αand IL-2 in peripheral blood and pleural effusion might have important clinical reference value in the differential diagnosis of tuberculous and malignant pleural effusion.
