摘要 肺炎克雷伯菌(Klebsiella pneumoniae,KP)作为主要的医院感染病原菌已被大家所熟知,同时它也是目前革兰阴性菌血流感染第二大菌。近二三十年间KP表现出两大趋势:其一是越来越多KP发展为多重耐药菌株甚至出现泛耐药菌株;其二是部分KP表现为高毒力且常侵袭健康的宿主并造成严重的侵袭性疾病,出现了高毒力肺炎克雷伯菌(Hypervirulent Klebsiella pneumoniae,HVKP)。现普遍认为造成HVKP高毒力的主要原因是其自身厚重的荚膜抗吞噬作用及其强大的摄铁系统。目前鉴定高毒力肺炎克雷伯菌尚无金标准,这对临床的早期诊断和治疗造成了较大的困难。HVKP除对氨苄西林天然耐药外,对其他大多数抗菌药敏感,但这几年产β-内酰胺酶的HVKP和产碳青霉烯酶的HVKP也陆续报道。本文对HVKP的研究进展做一综合阐述。 Klebsiella pneumoniae (KP) is familiar to us as the main pathogen of nosocomial infection and as the second-largest bloodstream infections bacteria in gram negative bacteria. There are two trends related to the development of KP recently, the one is more and more KP developed multidrug-resistance even pan-drug resistance, the other is some of the KP express hypervirulence, infecting healthy population and inducing severe invasive disease, suggesting that Hypervirulent Klebsiella pneumoniae(HVKP) has emerged. At present, some studies have indicated that the ability of anti-neutrophil phagocytosis and the iron acquisition systems are the two key reasons of high virulence in HVKP. Nevertheless, for there is no gold standard to identify HVKP presently, it makes the early diagnosis and treatment become more difficult. HVKP is sensitive to most of the antibiotics except for ampicillin, but there have also been some HVKPs producing β-lactamase and carbapenemases in recent years. This paper reviews the research progress of HVKP.
