摘要
目的:观察应用经皮穴位电刺激预防髋膝关节置换术后深静脉血栓的临床疗效。方法:收集本院骨科2013年3月至2016年4月行髋膝关节置换术后气虚血瘀证患者90例,按随机数字表法分为治疗组和对照组各45例。对照组采用低分子肝素钙皮下注射,治疗组采用经皮穴位电刺激,治疗14d后观察凝血功能、血流动力学指标变化情况和深静脉血栓发生情况,评定临床疗效。结果:治疗14d后,两组PT和APTT较治疗前明显延长,差异有统计学意义(P<0.01),组间差异无统计学意义(P>0.05);FIB和Ddimer较治疗前明显下降,差异有统计学意义(P<0.01),治疗组下降优于对照组,差异有统计学意义(P<0.01);两组股总静脉血管内径较治疗前明显增宽,差异有统计学意义(P<0.01),组间差异无统计学意义(P>0.05);股总静脉血流速度较治疗前明显增快,差异有统计学意义(P<0.01),治疗组增快优于对照组,差异有统计学意义(P<0.01);两组DVT阳性率差异无统计学意义(P>0.05)。结论:经皮穴位电刺激可改善髋膝关节置换术后(气虚血瘀证)患者血液高凝状态,降低深静脉血栓发生率。
Objective:To observe the clinical effect of percutaneous acupoint electrical stimulation on the prevention of deep venous thrombosis(DVT)after hip and knee arthroplasty.Methods:All 90 cases who were with Qi deficiency and blood stasis syndrome,after hip and knee arthroplasty,were randomly divided into treatment group and control group,with 45 cases in each group,from April 2016 to March 2013 in the department of orthopedics of our hospital.The control group using low molecular weight heparin subcutaneous injection,and the treatment group were treated with transcutaneous electrical acupoint stimulation.The index of coagulation function,hemodynamics and positive rate of DVT in both groups were observed two weeks after the treatment.Results:After treatment,the PT and APTT were significantly prolonged than the baseline(P〈0.01),and there was no significant difference between the two groups(P〈0.05).The FIB and Ddimer in both group were lower than pre-treatment(P〈0.01),and the treatment group was lower than the control group(P〈0.01).The vessel diameter of common femoral vein was obviously wider than pre-treatment(P〈0.01),and there was no significant difference between the two groups(P〈0.05).The blood flow velocity of common femoral vein was faster than pre-treatment(P〈0.01),and the treatment group was faster than the control group(P〈0.01).The positive rate of DVT was not significantly different between two groups(P〉0.05).Conclusion:Percutaneous acupoint electrical stimulation can improve the hypercoagulable state of the patients after the replacement of hip and knee joint(Qi deficiency and blood stasis syndrome)and reduce the incidence of DVT.
