摘要
目的研究探讨血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)在检测肺结核合并感染的老年患者中的诊断价值以及通过动态检测评判治疗及预后效果。方法选取我院2015年9月至2017年1月期间收诊的60例肺结核合并感染的老年患者为实验组(同时根据肺感染严重程度分成重症组37例与非重症组23例),30例单纯肺结核老年患者为肺结核组,另选择30例健康者为对照组。检测所有实验对象PCT、hs-CRP、IL-6浓度,对比分析PCT、hs-CRP、IL-6检测的灵敏度及特异度,诊断并评估患者病症相关性等。结果实验组PCT、hs-CRP、IL-6含量显著高于单纯肺结核组,肺结核组又显著高于健康对照组,重症组患者PCT、hs-CRP、IL-6含量显著高于非重症组,差异均具有统计学意义。PCT检测的特异度高于hs-CRP、IL-6,而hs-CRP检测的灵敏度高于PCT、IL-6。PCT、hs-CRP、IL-6含量与病症严重程度成正相关关系。结论 PCT、hs-CRP、IL-6检测可为肺结核合并感染的老年患者的诊断及判断病症严重程度提供较为准确的信息,另外三者的联合检测可提高检测灵敏度及特异度。
Objective To investigate the diagnostic value of the serum calcitonin（PCT）, hypersensitive c-reactive protein（hs-CRP） and interleukin 6（IL-6） in the elderly patients with pulmonary tuberculosis infection and evaluate the effect of treatment and prognosis by dynamic detection. Methods 60 cases of elderly patients with pulmonary tuberculosis infection in our hospital from September 2015 to January 2017 were selected for the experimental group meanwhile, according to the severity of lung infection, 37 cases of severe disease group and 23 cases of non-severe group, 30 cases of elderly patients with simple pulmonary tuberculosis were tuberculosis group, 30 cases of healthy subjects were chose as control group. The concentrations of PCT, hs-CRP and IL-6 were detected in all subjects, and the sensitivity and specificity of PCT, hs-CRP and IL-6 were analyzed, and the correlation of patients＇ symptoms was diagnosed and evaluated. Results The content of PCT, hs-CRP and IL-6 in the experimental group was significantly higher than that in the simple pulmonary tuberculosis group, and the tuberculosis group was significantly higher than that in the healthy control group, and the PCT, hs-CRP and IL-6 levels in the severe group were significantly higher than those in the non-severe group with statistical difference. The specificity of PCT detection was higher than that of hs-CRP and IL-6, while the sensitivity of hs-CRP detection was higher than that of PCT and IL-6. The content of PCT, hs-CRP and IL-6 was positively correlated with the severity of the disease. Conclusion The detection of PCT, hs-CRP and IL-6 can provide more accurate information for the diagnosis and severity of the elderly patients with pulmonary tuberculosis infection, and the combined test of the three can improve the sensitivity and specificity of detection.
