摘要
目的探讨影响恶性肿瘤患者早期肠内营养(EEN)疗效的相关因素。方法将接受EEN治疗的114例恶性肿瘤患者分为观察组(67例)与对照组(47例),观察组EEN达标,对照组EEN未达标,现回顾性分析上述研究对象的临床资料。结果单因素分析可见,两组患者血管活性药治疗、抗生素联合应用、镇静镇痛药治疗、机械通气治疗、合并低白蛋白血症、化疗史、肠内营养治疗史比较有统计学差异(P<0.05),而性别、年龄、体质指数(BMI)、幽门后喂养、消化道用药、放疗史、手术史无统计学差异(P>0.05)。多因素非Logistic回归分析可见,镇静镇痛药治疗、机械通气治疗、合并低白蛋白血症是影响恶性肿瘤患者EEN疗效的独立危险因素(P<0.05),肠内营养治疗史则是其独立保护因素(P<0.05)。结论影响恶性肿瘤患者EEN疗效的因素较多,严格把控镇静镇痛以及机械通气的治疗指征、纠正低白蛋白血症、避免不必要禁食可以改善恶性肿瘤患者EEN的疗效。
Objective To explore the factors related to the efficacy of early enteral nutrition（EEN） in the treatment of malignant tumor. Methods 114 cases of malignant tumor treated with EEN were divided into the observation group（67 cases） and the control group （47 cases）, and the observation group was up to standard, while the control group wasn＇ t up to standard, then their clinical data were analyzed retrospectively. Results The single-factor analysis showed that there were statistical differences in the vasoactive drugs, antibiotic combination, sedative and analgesic drugs, mechanical ventilation, hypoalbuminemia, chemotherapy history and enteral nutrition history between the 2 groups （ P 〉 0.05 ）, while there had no statistical differences in the gender, age, body mass index （BMI）, post pylorus feeding, digestive tract medication, radiotherapy history and operation histo- ry were found（ P 〈 0.05 ）. The multivariate non-Logistic regression analysis showed that the sedative and analgesic drugs, mechanical ventilation and hypoalbuminemia were independent risk factors related to the efficacy of EEN in the treatment of malignant tumor（ P 〈 0.05 ）, while the enteral nutrition history was an independent protective factors （ P 〈 0.05 ）. Conclusion There are many factors influencing the efficacy of EEN in the treatment of malignant tumor, and controlling the indication of sedation, analgesia and mechanical ventilation strictly, correcting hypoalbuminemia, avoiding unnecessary fasting can improve the efficacy of EEN in patients with malignant tumor.
