摘要
目的:探讨CT引导下经皮肺穿刺活检术并发症发生情况及其危险因素。方法:收治行CT引导下经皮肺穿刺活检术患者118例,观察患者术后并发症发生情况及危险因素。结果:所有患者均穿刺成功,均没有出现大量出血、空气栓塞、胸痛、感染和死亡等并发症,但出现气胸36例,出血25例;穿刺活检术后并发症主要是出血和气胸,出血危险因素主要是年龄、穿刺次数和时间、病灶大小和深度,气胸危险因素主要是年龄、穿刺次数和时间、病灶大小和深度、有无慢性肺部病变(P<0.05)。结论:CT引导下经皮肺穿刺活检术并发症主要是气胸和出血,其危险因素是年龄>60岁、穿刺次数和时间较长、病灶较小和较深,气胸还与慢性肺部病变有关,应注意避免并发症发生。
Objective:To explore the complications and risk factors of percutaneous transthoracic needle biopsy under CT guidance.Methods:118 patients with percutaneous transthoracic needle biopsy under CT guidance were selected,and we observed the incidence of postoperative complications and risk factors.Results:All patients were successfully punctured without complications such as massive bleeding,air embolism,chest pain,infection and death,but there were 36 cases of pneumothorax and 25 cases of bleeding.The main complications of puncture biopsy were bleeding and pneumothorax,the main risk factors of hemorrhage were age,number and time of puncture,size and depth of lesion,the main risk factors of pneumothorax were age,number and time of puncture,size and depth of lesion and chronic lung disease(P<0.05).Conclusion:The main complications of percutaneous transthoracic needle biopsy under CT guidance were pneumothorax and hemorrhage,the risk factors were age more than 60 years,longer number and time of puncture,smaller and deeper lesion,pneumothorax was also associated with chronic lung disease,we should pay attention to avoid complications.
