摘要
目的探析雷珠单抗联合激光疗法治疗视网膜静脉阻塞继发黄斑水肿的有效性。方法回顾性分析2015年12月至2016年12月,视网膜静脉阻塞继发黄斑水肿30例患者的临床资料,采用雷珠单抗联合激光疗法进行治疗。患眼经玻璃体腔注射雷珠单抗后10 d,根据患者视网膜发生病变的情况选择激光的波长、次数和能量进行光凝治疗。所有患者均行荧光素眼底血管造影(Fluorescein fundus angiography,FFA)、光学相干断层扫描(Optical coherence tomography,OCT)、最佳视力矫正(Best corrected visual acuity,BCVA)检查。观察患眼中央视网膜厚度(Central retinal thickness,CRT)和BCVA改变情况,其中BCVA判定采用最小分辨角对数(Logarithm of the minimum angle of resolution,logMAR)进行比较分析。结果 30例患者治疗后logMAR的平均值(0.45±0.30),较治疗前平均值(0.73±0.12)明显改善,差异具有统计学意义(P<0.05);而其CRT平均值为(325.58±10.17)μm,较治疗前平均值(520.10±10.22)μm明显改善,差异具有统计学意义(P<0.05)。结论对于视网膜静脉阻塞继发性黄斑水肿患者而言,予用雷珠单抗联合激光进行治疗,不仅可以有效改善其临床症状,校正视力,而且安全可靠,因而值得临床作进一步的推广与应用。
Objective To analyze the effect of joint therapy of laser and Lucentis on macular edema secondary to retinal vein obstruction. Methods Totally 30 patients diagnosed macular edema secondary to retinal vein obstruction and treated from Dec. 2015 to Dec. 2016 were selected and analyzed retrospectively. All these patients were given fluorescein fundus angiography ( FFV), light dry tomography (OCT) and best vision correction (BCVA) checks and the joint therapy of laser and Lucentis. The final resuh converted to logMAR (minimum resolution Angle logarithmic) and central retinal thickness (CRT) before and after the treatment were observed and com- pared. Results After the treatment, the logMAR of the patients was (0. 45 ± 0. 30), which was (0. 73 ±0. 12) before the treatment, and the gap was statistically significant ( P 〈 0. 05 ). The CRT was ( 325.58 ± 10. 2 ) μm, which was (520. 10± 10.2 )μm before the treatment, also indicating a difference of statistical significance ( P 〈 0. 05 ). Conclusions For patients with macular edema secondary to retinal vein occlusion, joint application of laser therapy and Lucentis, will settle the clinical symptoms and correct vision safety, reliably, effectively and should be further popularized and applied clinically.
