Retention Enema Efficacy in Radiation Proctitis Using Recombinant Human Granulocyte Macrophage Colony-Stimulating Factor

摘要 目的评价重组人粒细胞-巨噬细胞集落刺激因子(rh GM-CSF)治疗放射性直肠炎的效果。方法将60例放射性直肠炎患者随机分为治疗组和对照组,治疗组31例,对照组29例,分别给以rh GM-CSF+地塞米松注射液+利多卡因针+凝血酶冻干粉+甲硝唑液(治疗组)和地塞米松注射液+利多卡因针+凝血酶冻干粉+甲硝唑液(对照组)灌肠治疗。从腹痛的程度、腹泻是否严重、是否有血便等肠道表现对两组患者进行比较,判断临床症状减轻的程度。结果治疗组治愈率为54.8%(17/31),对照组治愈率为24.1%(7/29),两组比较差异有统计学意义(X^2=4.67,P=0.03)。治疗组总有效率为83.9%(26/31),对照组总有效率为55.2%(16/29),两组比较差异有统计学意义(X^2=4.59,P=0.03)。结论 rh GM-CSF+地塞米松注射液+利多卡因针+凝血酶冻干粉+甲硝唑液保留灌肠治疗放射性直肠炎疗效确切,安全有效。 Objective To evaluate the effect of recombinant human granulocyte - macrophage colony stimulating fac- tor ( rhGM - CSF) in the treatment of radiation proetitis. Methods Sixty patients with radiation proctitis, 19 males and 41 females were randomly divided into experimental group and control group. The experimental group was treated with rhGM - CSF + dexamethasone injection + lidocaine + lyophilizing thrombin powder + metronidazole and the control group was treated wiht dexamethasone injection + lidocaine + lyophilized thrombin powder + metronidazole. The therapy efficiency of two groups from the degree of abdominal pain, diarrhea, bloody stools and tenesmus were Compared. Results The cure rate was 54.8% ( 17/31 ) in the treatment group and 24.1% (7/29) in the control group. The difference was statistically signifi- cant (X^2 = 4.67, P = O. 03). The total effective rate was 83.9% (26/31) in the treatment group and 55.2% (16/29) in the control group. There was significant difference between the two groups (X^2 = 4.59, P = 0.03 ). Conclusion Retention enema efficacy in radiation proetitis using rhGM - CSF + dexamethasone injection + lidocaine + lyophilizing thrombin pow- der + metronidazole was safe and effective.
