摘要
目的:探讨依托咪酯复合丙泊酚方案对无痛胃肠镜检查过程中应激及炎症反应的影响。方法:380例在本院接受无痛胃肠镜检查者作为研究对象,经随机数表法分为对照组、观察组各190例。对照组接受丙泊酚静脉麻醉,观察组接受依托咪酯复合丙泊酚静脉麻醉。对比两组研究对象检查前24h、检查过程中、检查后1h血清中应激指标、炎症因子含量的差异。结果:检查前24h,两组研究对象血清中应激指标、炎症因子含量的差异无统计学意义。检查过程中及检查后1h,两组研究对象血清中NE、E、Cor、ALD、AT-Ⅱ、IL-1β、IL-4、IL-6、IL-8、IL-10、TNF-α的含量高于检查前24h;且观察组血清中NE、E、Cor、ALD、AT-Ⅱ、IL-1β、IL-4、IL-6、IL-8、IL-10、TNF-α的含量均低于对照组。结论:无痛胃肠镜检查采用依托咪酯复合丙泊酚静脉麻醉,可有效缓解检查过程中及检查后早期的应激及炎症反应。
Objective:To study the effect of etomidate combined with propofol on stress and inflammatory response in painless gastrointestinal endoscopy.Methods:380 subjects who received painless gastrointestinal endoscopy in the hospital were selected as the research subjects and divided into control group and observation group by random number table,190 cases in each group.Control group received propofol intravenous anesthesia,and observation group received etomidate combined with propofol intravenous anesthesia.The differences in serum levels of stress indexes and inflammatory factors were compared between the two groups 24 hbefore examination,during examination and 1 hafter examination.Results:24 hbefore examination,difference in serum levels of stress indexes and inflammatory factors were not statistically significant between the two groups of subjects(P>0.05).During examination and 1 hafter examination,serum NE,E,Cor,ALD,AT-Ⅱ,IL-1β,IL-4,IL-6,IL-8,IL-10 and TNF-αcontents of both groups were higher than those 24 hbefore examination(P<0.05),and serum NE,E,Cor,ALD,AT-Ⅱ,IL-1β,IL-4,IL-6,IL-8,IL-10 and TNF-αcontents of observation group were lower than those of control group(P<0.05).Conclusion:Etomidate combined with propofol for painless gastrointestinal endoscopy can effectively alleviate the stress and inflammatory response during and early after the examination.
