自体耳软骨游离移植矫治重度乳头内陷的临床应用 被引量:8

Clinical application of autologous auricular cartilage transplantation in the correction of severe crater nipple

摘要 目的探讨应用自体耳软骨游离移植矫治重度乳头内陷的方法及疗效。方法自2012年7月至2015年7月,对收治的12例(21侧)重度乳头内陷患者,在乳晕上设计4个区,对称、等大、菱形;切除菱形区内的乳晕皮肤,在下象限内的菱形区内彻底松解切断乳头下挛缩纤维束及平滑肌,将取出的耳甲腔软骨修剪成圆柱形,植于乳头内及乳头基底部,并用乳晕下的乳腺组织将软骨包裹;乳头基底部采用荷包缝合,然后在合适的位置上固定圆柱形软骨,切口分层缝合。结果术后所有患者获随访6~15个月,切口均一期愈合。无乳头坏死发生,无麻木感,乳头勃起功能正常,乳头凸度及外形满意,触感具有乳头组织的特有韧性;后期切口瘢痕不明显,无复发。结论采用自体耳软骨游离移植是治疗重度乳头内陷的有效方法之一。 Objective To explore the method and efficacy of autologous auricular cartilage-free transplantation in the correction of severe crater nipple. Methods From July 2012 to July 2015, 12 cases(21 sides) with severe crater nipples were involved in this study. Four symmetric and equal rhombus regions were designed in the mammary areola. The areola skin was removed within the rhombus regions. Contracture fiber bundle and smooth muscle under the nipple in the rhombus region within the lower quadrant was thoroughly released. An incision was made following the long diagonal, then the concha cartilage was trimmed into a cylindrical shape and transplanted to the nipple and the base of the nipple. The concha cartilage was encased in mammary tissue under the areola. Purse string suture was performed to fix the position of cylinder cartilage and the nipple base was tightened. Finally, the incisions were sutured layer by layer. Results All patients were followed up from 6 to 15 months. All incisions were primarily healed. No nipple necrosis or numbness occurred. The erection of the nipple was normal. The nipple convexity and appearance was satisfactory. The tactile impression had the unique tenacity of papillary tissue. The incision scar was not obvious and there was no recurrence. Conclusion Autologous auricular cartilage free transplantation can be used as one of the effective methods to correct severe crater nipple.
