摘要
目的对肺癌患者全肺切除术后并发心律失常的原因进行分析,并提出相关护理对策。方法选择2015年3月~2017年3月在我院接受全肺叶切除术的肺癌患者68例,根据患者术后是否发生心律失常分为对照组和观察组,观察组为术后发生心律失常患者12例,对照组为术后未发生心律失常患者56例,统计两组患者相关数据,分析术后并发心律失常的原因,并提出对应的护理对策。结果全肺叶切除术后心律失常发生的原因与年龄、吸烟史、肺功能受损史、心脏病史、手术时间、术中出血量、术后低氧血症、术后血液动力学改变、术后纵膈摆动、术后剧烈疼痛、术后便秘有相关性,且差异有统计学意义(P<0.05);经Logistic回归分析,全肺叶切除术后心律失常的独立危险因素有:年龄>65岁、肺功能受损史、心脏病史、术后低氧血症、术后血液动力学改变,且差异有统计学意义(P<0.05)。结论肺癌患者全肺切除术后并发心律失常的原因较多,包括年龄、心肺功能、手术类型、术中出血量等,对此,需要加强对患者心电监护,及时止痛、发现并处理并发症,以减少心律失常的发生。
Objective To analyze the causes of arrhythmia after total lobectomy in patients with lung cancer, and put forward relevant nursing countermeasures.Methods 68 patients with lung cancer with total lobectomy from March 2015 to March 2017 of our hospital were selected as study objects.Patients were divided into groups according to whether or not there was arrhythmia after the operation.12 patients with arrhythmia after operation were selected as observation group.56 patients without arrhythmia after operation were selected as control group. The related data of two groups were analyzed, and the causes of arrhythmia after operation were analyzed, and the corresponding nursing countermeasures were put forward.Results The reason of arrhythmia after total lobectomy was related with age, smoking history, history of pulmonary function impairment, heart history, operation time, intraoperative blood loss, postoperative hypoxemia, postoperative hemodynamic changes, postoperative mediastinal swing, postoperative severe pain and postoperative constipation （P〈0.05）.Logistic regression analysis showed that the independent risk factors of arrhythmia after total lobectomy were aged more than 65 years old,history of pulmonary function impairment, heart history, postoperative hypoxemia and postoperative hemodynamic changes （P〈0.05）.Conclusion The reason is more complicated with arrhythmia after pneumoneetomy for lung cancer patients, including age, cardiopulmonary function, type of surgery, intraoperative blood loss.In this regard, strengthen the patients ECG, timely analgesia, timely detection and treatment complications need to take to reduce the incidence of arrhythmia.
