摘要
目的探讨氟达拉滨联合BEAM方案预处理自体外周血干细胞移植治疗非霍奇金淋巴瘤患者的临床效果。方法非霍奇金淋巴瘤患者55例据人院序号分为两组，分别给予BEAM方案（卡莫司汀＋阿糖胞苷＋马法兰）预处理（对照组27例）及加用氟达拉滨治疗（观察组28例）。此后均给予自体外周血干细胞移植治疗，对比两组患者治疗效果。结果观察组患者移植后白细胞最低值、粒细胞计数小于0．5×10^9儿的下降时间及恢复正常时间均低于对照组（P〈0．05）。观察组患者移植治疗后外周血淋巴细胞总数与治疗前相比稍有降低，差异无统计学意义（P〉0．05），但移植后CD4^＋淋巴细胞总数与治疗前相比降低，自然杀伤淋巴细胞总数与治疗前相比增加，差异有统计学意义（P〈0．05）。两组患者治疗缓解率差异无统计学意义（P〉0．05），随访期间复发率差异有统计学意义（P〈0．05）。结论氟达拉滨联合BEAM预处理自体外周血干细胞移植治疗非霍奇金淋巴瘤患者效果良好。
Objective To investigate the effect of fludarabine combined with BEAM regimen in pretreatment of autologous peripheral blood stem cell transplantation in patients with non-Hodgkin lymphoma. Methods 55 patients with non Hodgkin lymphoma were given BEAM regimen （carmustine Ara-C pretreatment （Ma Falan）, control group n = 27） and fludarabine （observation group, n = 28）. Treatment effects were compared between the two groups. Results The observation group of patients after transplantation in the treatment of white blood cell minimum down time less than 0.5 × 10^9/L and granulocyte returned to normal time and the control group there were significant differences compared with patients and lower than that of control group （ P 〈 0. 05）. The total number of observation and treatment of peripheral blood lymphocytes of patients after transplantation in the treatment group than before although slightly lower but No statistical significance （ P 〉 O. 05）, but the total number of CD4 lymphocytes after transplantation compared with before treatment decreased, NK cells （natural killer cells） total compared with before treatment increased significantly （ P 〈 0.05 ）. Two groups of patients with remission rate showed no significant difference （ P 〉 0. 05）. Two groups of patients with recurrence rate compared with significant difference （ P 〈 0.05）. Conclusion Fludarabine combined with BEAM pretreatment type peripheral blood stem cell transplantation in the treatment of patients with non-Hodgkin lymphoma is good.
