摘要
颅内动脉瘤是严重危害人类健康的疾病,高发的动脉瘤破裂性蛛网膜下腔出血后脑血管痉挛的致死致残率亦非常高。颅内动脉瘤夹闭术对于破裂与否的动脉瘤均是有效的治疗方法,但动脉瘤破裂性蛛网膜下腔出血患者术后易发生脑血管痉挛。星状神经节阻滞术(SGB)在颅内动脉瘤夹闭术中不但可以在术中发挥保持脑保护的作用,还可以降低脑血管张力,改善阻滞侧脑循环、改善脑血流、缓解脑血管痉挛,进而逆转动脉瘤性蛛网膜下腔出血后脑血管痉挛所致的延迟性缺血性损伤,而超声引导SGB使SGB定位更加精准,用药剂量更小,术中血管和神经损伤更少,更适合临床应用,发展前景非常乐观。
Intracranial aneurysms are very harmful diseases to human health, and the lethality and morbidity of cerebral vasospasm after aneurysmal subarachnoid hemorrhage with high incidence are very high. Intracranial aneurysm clipping is an effective treatment for both ruptured and unruptured aneurysm, but cerebral vasospasm may easisly occur in patients with subarachnoid hemorrhage of ruptured aneurysm after surgery. The role of stellate ganglion block(SGB) in intracranial aneu- rysm clipping is not only brain protection during surgery, but also reducing the cerebrovascular tension,improving the block lateral cerebral circulation, improving cerebral blood flow, relieving cerebral vasospasm, and reversing delayed ischemic injury caused by cerebral vasospasm following aneurysmal subarachnoid hemorrhage. The ultrasound-guided SGB makes SGB positioning more accurate, enables less anesthetic dose, less intraoperative blood vessels and nerve damage, therefore is more suitable for clinical applications, with very optimistic development prospect.
