摘要
目的本研究的目的是运用CT扫描图像分析急性胰腺炎的远期后遗症。方法回顾性地分析了85例患者,患者在CT图像上诊断为急性胰腺炎,在急性发作至少1个月后接受了1次或多次随访CT扫描,我们把所有随访CT扫描的结果制成表格,包括初次急性胰腺炎发作的时间间隔。结果 16.5%的患者有残存的炎症改变,距离急性胰腺炎初次发作的中位数间隔时间是78天。27.1%的患者有残存的积液,且持续存在的中位数时间是132天。4例患者有残存的实质样炎性包块,很容易被误认为是肿瘤。其它远期后遗症包括胰腺导管扩张、胰腺萎缩、新发或增强的胰腺钙化、胆道扩张也做出分析。图片分析显示了这些残存改变和远期并发症。结论识别急性胰腺炎的残存改变,其中一些是长期存在的,对于正确解读胰腺CT来说十分重要。这些残存改变与急性胰腺炎或胰腺/胰周肿瘤很相像,可能会引起误诊,尤其是在没有之前CT图像的情况下。
Objective To analyze late sequelae following acute pancreatitis using CT scan imaging. Methods 85 patients who were diagnosed with acute pancreatitis on CT imaging were retrospectively analyzed. They received at least one or more followup CT scans 1 month after acute onset. The results of CT scan were tabulated,including the interval from the onset of initial acute pancreatitis. Results 16. 5% of patients had residual inflammatory changes,with median interval of 78 days from the onset of initial acute pancreatitis. 27. 1% of patients had residual effusion,with median duration of 132 days. 4 patients had residual parenchymal inflammatory mass,which tended to be mistaken as tumors. Other late sequelae included pancreatic duct expansion,pancreas atrophy,new or enhanced pancreatic calcification and bile duct expansion,which were also analyzed. The imaging analysis revealed these residual changes and long-term complications. Conclusion To identify the residual changes in acute pancreatitis,some of which are long-term,is very important in correct interpretation of the CT of pancreas. These residual changes are very similar to acute pancreatitis or pancreatic/peripancreatic tumors,which may lead to misdiagnosis,especially if there is no previous CT image.
