青稞米对空腹血糖受损患者血糖波动的影响及其相关因素分析 被引量:6

Influence of highland barley rice on glucose fluctuation in patients with impaired fasting glucose and its correlative factors analysis

摘要 目的对空腹血糖受损(impaired fasting glucose,IFG)患者进食富含β-葡聚糖的青稞米粥后血糖波动及其相关因素进行研究,为有效指导IFG患者饮食治疗提供可靠依据。方法从武警总医院1个月内体检人员中筛选出IFG患者36例,采用自身对照的方法,分别使患者进食含有等量碳水化合物的大米粥(市售大米组)和青稞米粥(青稞原米组),并测定空腹血糖及餐后15 min、30 min、45 min、60 min、90 min、120 min后的血糖情况。观察分析食用青稞米粥和大米粥后血糖的波动情况;并对食用青稞米粥后不同血糖波动的患者进行相关因素(如年龄、性别、腰围、体重指数、高血压及吸烟等)分析。结果 (1)青稞原米组餐后15 min、30 min、45 min、60 min、90 min血糖均值高于空腹时,差异均有统计学意义(P<0.008);市售大米组各个时间点血糖均值高于空腹时,差异均有统计学意义(P<0.008)。同一时间点两组间血糖均数比较:空腹时两组间血糖值差异无统计学意义(t=1.544,P=0.129);餐后各时间点市售大米组血糖值均高于青稞原米组,差异有统计学意义(P<0.008)。(2)青稞原米组餐后15 min与90 min血糖波动值比较,差异无统计学意义(P=0.970);其余各时间点之间差异均有统计学意义(P<0.008);市售大米组各时间点之间差异均有统计学意义(P<0.008);两处理组之间不同时间点血糖波动值比较,差异均有统计学意义(P<0.008)。(3)年龄>50岁、女性、腹型肥胖及吸烟史患者食用青稞米后血糖波动更明显。结论对于IFG的人群,进食含等量碳水化合物的青稞米能够明显减少餐后血糖的升高和波动幅度。而在年龄>50岁、女性、腹型肥胖及吸烟史可能是影响餐后血糖波动的因素。 Objective The objective of this study was to investigate the influence of highland barley rice(rice in beta glucan) on glucose fluctuation and the correlative factors in patients with impaired fasting glucose(IFG) and to provide scientific evidence to effectively instruct the dietary treatment of patients with IFG. Methods A total of 36 cases were selected from the Center of Health Examination in General Hospital of Chinese People's Armed Police Force in one month. Patients were given rice gruel(market rice group) and highland barley rice porridge(highland barley rice group) respectively by means of self-reference approach. The blood glucose of the two groups were respectively measured before consumption and at the time point of 15, 30, 45, 60, 90 and 120 minutes after consumption. The glucose fluctuations of two groups were observed after having rice gruel and highland barley rice porridge; The correlative factors inducing glucose fluctuation after having highland barley porridge, such as age, gender, waist circumference, body mass index(BMI), hypertension and smoking were analyzed. Results(1) The blood glucose of highland barley rice group at the time point of 15, 30, 45, 60, and 90 minutes after having highland barley rice were higher than before consumption, and the difference was statistically significant(P0.008). The blood glucose of the market rice group at each time point was higher than before consumption, and the difference was statistically significant(P0.008). The blood glucose of two groups were compared at the same time points: there was no significant difference before consumption(t=1.544, P=0.129), while the blood glucose at each time point after consumption in market rice group was higher than that the highland barley rice group, and the differences were statistically significant(P0.008).(2) The difference of blood glucose fluctuation at the time point of 15 min and 90 min after having highland barley rice was not statistically significant(P=0.970), while the differences between the remaining time points were statistically significant(P0.008). The differences between all the time points in market rice group were statistically significant(P0.008). Blood glucose fluctuation at different time points between the two groups werecompared, and the differences were statistically significant(P0.008).(3) Blood glucose fluctuation after having highland barley rice in patients with age 50, female, abdominal obesity and smoking history were more obvious. Conclusions For the IFG population, eating highland barley rice can significantly reduce postprandial glucose levels and fluctuations. While the factors of age 50, female, abdominal obesity and smoking history may adversly affect postprandial blood glucose fluctuation.
