脐动脉血血气pH值与Apgar评分在新生儿窒息诊断中的比较研究 被引量:14

摘要 目的探讨在新生儿窒息临床诊断中各诊断指标的利用价值,以为及时采取预防措施提供理论基础。方法选取其中50例窒息新生儿为研究组,262例健康新生儿为对照组,检测分娩1分钟Apgar评分,采集脐动脉血进行血气p H值分析,对研究组进行复苏后转入儿科病房观察有无脏器损伤和进行鉴别诊断,并进行统计学分析。结果脐动脉血p H检测结果显示,对照组p H值不正常新生儿(p H<7.2)的比率为5.0%,而研究组p H值不正常新生儿均大于70%;比较Apgar评分和脐动脉血血气p H值在新生儿窒息诊断的特异性和灵敏性时发现,Apgar评分的特异度和敏感度分别为93.6%和71.1%,脐动脉血p H值的特异度和敏感度分别为99.3%和93.3%;比较Apgar评分和脐动脉血血气p H值与脏器官损伤程度的关系可以发现,随着Apgar评分和脐动脉血血气p H值下降,脏器管损伤率逐渐上升的趋势。结论在新生儿窒息诊断中,脐动脉血血气p H值比Apgar评分特异性更好,敏感度更高,在临床上Apgar评分和脐动脉血血气p H值结合起来有助于提高对新生儿窒息诊断的准确率。 Objective:Study the use value of various diagnostic index for clinical diagnosis of neonatal asphyxia,and provide effective preventive measures timely for doctor. Methods 50 asphyxia newborns as research group,262 healthy newborns as control group,Test 1 min Apgar score,blood gas p H analysis of umbilical arterial blood,the research group was transferred to the pediatric ward for observing whether dirty organ was damaged after recovery,do statistical analysis using collected data. Results:We found that 5.0% Control group newborns have an unnormal p H value(p H 〈 7.2),and the experimental group are more than 70%;Compare the specificity and sensitivity of Apgar score and umbilical arterial blood flesh p H in neonatal asphyxia diagnosis. we found that the specific degrees and the sensitivity of Apgar score is 93.6% and 71.1% respectively,umbilical artery blood p H is 99.3% and 93.3% respectively;Compare the relationship of Apgar score and umbilical arterial blood flesh p H and dirty organ damage degree. We can found,along with Apgar score and umbilical arterial blood flesh p H decline,viscera pipe damage rate gradually rising. Conclusion:In neonatal asphyxia diagnosis,umbilical artery blood gas p H value is better than Apgar score specificity,sensitivity is higher,Combined the Apgar score and umbilical artery blood gas p H value can improve the accuracy of diagnosis In neonatal asphyxia diagnosis.
