摘要
目的探究骨劈开术在口腔种植骨增量中的应用效果。方法选取该院收治的60例种植区骨量不足或牙槽嵴高度充足但厚度不足的患者作为观察对象,所有患者的入院时间在2015年3月—2016年12月。将观察对象采用抽签法随机分为观察组和对照组,每组30例患者。对照组患者采用常规骨劈开术来进行治疗,观察组患者采用新研发的3D模板引导H型骨劈开术来进行治疗。治疗完成后观察分析两组患者的唇舌向牙槽厚度、唇侧骨板向唇侧的移动幅度、唇侧骨板移动量、患者对修复效果的满意度、随访发生异常的情况等,通过对比分析来了解不同术式应用在口腔种植骨增量中的效果。结果手术前,两组患者的唇舌向牙槽厚度、唇腭侧骨板的移动幅度、唇腭侧骨板移动量进行比较,得出的结果差异无统计学意义(P>0.05),术后,观察组治疗后患者的唇舌向牙槽厚度为(6.3±0.8)mm,骨板向唇腭侧的移动幅度为(6.6±0.9)mm,唇腭侧骨板移动量为(1.5±0.4)mm,而采用常规骨劈开术治疗的患者,治疗后的唇舌向牙槽厚度为(5.2±0.7)mm,骨板向唇腭侧的移动幅度为(5.7±0.8)mm,唇腭侧骨板移动量为(0.8±0.3)mm,两组间各项指标进行比较的结果差异有统计学意义(P<0.05);观察组患者中,对修复效果表示满意的患者有28例,占93.3%,对照组中,对修复效果表示满意的患者有21例,占70.0%,两组患者的满意度进行比较得出的差异有统计学意义(P<0.05);随访12~18个月,观察组中无人出现异常,对照组中出现异常的有4例,两组患者异常情况总发生率比较的得出的结果差异有统计学意义(P<0.05)。结论骨劈开术拓宽种植适应症,减少了骨移植手术机会,保证种植体植入的位置、方向及唇侧骨板的厚度,保证种植的长期效果,牙槽突唇侧骨板向唇侧移位,有利于支持及恢复缺牙区软组织外型,保证种植修复体周软组织的美学效果。
Objective To study the application effect of osteotomy in the dental implant bone augmentation. Methods60 cases of patients with bone inadequacy in the implantation region or adequate residual ridge height but thickness inadequacy admitted and treated in our hospital from March 2015 to December 2016 were selected and randomly divided into two groups with 30 cases in each, the control group adopted the routine osteotomy, while the observation group adopted the new 3D template to guide the H type osteotomy, after treatment, the faciolingual alveolus thickness, amplitude of the movement, bony plate movement amount, satisfactory degree to rehabilitation effect and occurrence of follow-up abnormalities were compared between the two groups, and the effect of different operation methods in the dental implant bone augmentation was compared. Results Before operation, there were no obvious differences in the faciolingual alveolus thickness, amplitude of the movement and bony plate movement amount(P>0.05), after operation, the differences in the faciolingual alveolus thickness, amplitude of the movement and bony plate movement amount between the observation group and the control group were statistically significant[(6.3±0.8),(6.6±0.9),(1.5±0.4)mm vs(5.2±0.7),(5.7±0.8),(0.8±0.3)mm](P<0.05), in the observation group, 28 cases were satisfied with the repair effect, accounting for 93.3%, in the control group, 21 cases were satisfied with the repair effect, accounting for 70.0%, and the difference in the satisfactory degree between the two groups was obvious(P<0.05), after 12-18 month follow-up, there were no abnormal patients in the observation group, and there were four abnormal cases in the control group, and the difference in the total incidence rate of abnormal situation between the two groups was statistically significant(P<0.05).Conclusion The osteotomy in broadening the implantation indications can reduce the bone implantation surgery opportunities, ensure the implantation site, direction and thickness of implant and long-term effect of implant, which is conducive to supporting and recovering the soft tissue profile in the edentulous area and ensure the aesthetic effect of implant repairsome soft tissue.
