旋髂浅动脉穿支皮瓣修复颊癌术后缺损的临床应用 被引量:3

Free Superficial Circumflex Iliac Artery Perforator Flap for Reconstruction of Soft Tissue Defects Secondary to Resection of Buccal Carcinoma

摘要 目的:探讨旋髂浅动脉穿支皮瓣修复颊癌根治术后颊部缺损的临床效果。方法:2014年1月~2014年6月,修复6例颊癌根治术后颊部缺损,所有患者术前均进行CT血管造影及彩色多普勒超声检查。结果:皮瓣面积30~63cm2,动脉管径0.6mm,静脉管径1.1mm,动静脉血管蒂分别为6.9cm和7.3cm,6例皮瓣全部成活,术后颊部形态良好,张口正常。结论:旋髂浅动脉穿支皮瓣质地柔软,穿支血管蒂较长,供区瘢痕隐蔽患者术后颊部外形丰满,张口功能恢复良好,是修复颊癌术后缺损的良好选择。 Objective: To investigate the therapeutic effect of free superficial circumflex iliac artery perforator (SCIP) flap for reconstruction of soft tissue defects secondary to resection of buccal carcinoma. Methods: From January 2014 to June 2014, six patients with buccal carcinoma received radical resection and immediate reconstructed with SCIP flap. CTA and color Doppler sonography were routinely performed before the surgery. Results: The flap sizes ranged from 30 to 63 cm2. The mean diameter of the superficial circumflex iliac arteries was 0.6 mm and of the veins was 1.1 mm. The mean arterial pedicle length was 6.9 cm, and the venous pedicle length was 7.3 cm. The mean reduction of mouth opening was 0.4 cm. No flap failure was found, and a satisfactory aesthetic outcome was obtained in all cases. Conclusion: The SCIP flap is a reliable, thin and pliable flap with long vascular pedicles and hidden donor site, and can achieve satisfactory postoperative buccal contour and mouth opening, making itself a good option for buccal defect reconstruction
