摘要
目的:了解山东能源新汶矿业集团莱芜中心医院(以下简称"我院")Ⅰ类切口手术围术期预防性应用抗菌药物的情况及变化趋势。方法:调取2012—2016年我院抗菌药物临床应用专项整治活动以来的Ⅰ类切口手术7 012例,对相关数据进行统计分析,并评价预防性用药的合理性。结果:7 012例Ⅰ类切口手术主要来自外科及骨科手术科室。2012—2016年,我院Ⅰ类切口手术围术期预防性应用抗菌药物的各项指标逐年趋于合理,其中抗菌药物预防性使用率由82.93%(865/1 043)降至29.76%(539/1 811);人均预防性用药疗程由166.32 h缩短至20.88 h;预防性给药时间合理率由69.13%(598/865)上升至97.78%(527/539);抗菌药物品种选择合理率由56.20%(486/865)升至86.27%(465/539);2014年后,我院Ⅰ类切口手术围术期无预防性联合用药情况。结论:通过干预,我院Ⅰ类切口手术围术期预防性应用抗菌药物存在的不合理现象得到了初步改善,合理预防性用药水平明显提高,但仍有不足之处,需继续努力。
OBJECTIVE： To investigate the preventive application and its changing trends of antibiotics in type Ⅰincision during perioperative period in Laiwu Central Hospital Of Shandong Energy Xinwen Mining Group（ hereinafter referred to as ＂our hospital＂）. METHODS： 7012 cases with clinical application of antibiotics in type Ⅰ incision in our hospital from 2012 to 2016 since the special rectifications were extracted,statistical analysis and evaluation on rationality of drug application were conducted. RESULTS： Surgery and orthopedic operation were the mainly involved departments in 7 012 cases of type Ⅰ incision operation. From 2012 to 2016,indices of preventive application of antibiotics in type Ⅰ incision in our hospital tended to rational year by year,the preventive application rate of antibiotics had decreased from 82. 93%（ 865/1 043） to 29. 76%（ 539/1 811）; the average course of preventive treatment had been shorten from 166. 32 h to 20. 88h; the rationality rate of preventive drug administration time had risen from 69. 13%（ 598/865） to 97. 78%（ 527/539）; the rationality rate of antibiotics selection had risen from56. 20%（ 486/865） to 86. 27%（ 465/539）; after 2014,no preventive drug combination in type Ⅰ incision during perioperative period was found in our hospital. CONCLUSIONS： The irrational preventive application of antibiotics in type Ⅰ incision during perioperative period in our hospital has been preliminarily improved, the rationality of preventive medication had been promoted significantly,while there are still some problems need to be solved.
