摘要
目的观察短期强化血糖控制对重症感染合并应激性高血糖患者血糖变异性(GV)及预后的影响。方法选取2011-02—2016-11收治的重症感染合并应激性高血糖患者90例,采用随机数字分组法将其分为两组。对照组45例,采用常规疗法;研究组45例,采用短期强化血糖控制。结果两组患者入院第1天空腹血糖(FPG)比较,差异无统计学意义,研究组患者入院第3天和第7天FPG水平明显低于对照组;研究组患者血糖达标时间、GV及28 d病死率均明显低于对照组;两组患者低血糖发生率比较,差异无统计学意义;存活患者与死亡患者在入院后第3天、第7天FPG水平、血糖达标时间及GV等方面差异有统计学意义;在入院第1天FPG水平及预后低血糖发生率差异无统计学意义。结论短期强化血糖控制对重症感染合并应激性高血糖患者血糖变异性及预后效果显著。
Objective To observe the effect of short-term intensive glycemic control on blood glucose variability (GV) and its prognosis on patients with severe infection complicated with stress hyperglycemia.Methods 90 patients with severe infection complicated with stress hyperglycemia in our hospital from February 2011 to November 2016 were randomly divided into two groups.45 cases in the control group were treated with routine therapy while 45 cases in the study group were treated with short-term intensive glycemic control. Results The first fasting abdominal blood glucose(FPG) in the two groups were compared and the difference was not statistically significant.The levels of FPG in the study group at day 3 and day 7 were significantly lower than those in the control group at day 3 and day 7 af- ter admission.The level of blood glucose, GV and the mortality of 28 days in the study group were significantly lower than those in the control group.The incidence of hypoglycemia in the two groups was not statistically significant.The fasting blood glucose level, blood glucose standard-reaching time and GV of survived and dead patients on day 3 and 7 after admission showed significant difference.There was no significant difference in the level of blood glucose and the incidence of hypoglycemia at the first day of admission.Conclusion Short-term intensive glycemic control has a significant effect on blood glucose variability and good prognosis in patients with severe infection complicated with stress hyperglycemia.
