摘要 目的观察二期人工晶体植入术中,超声生物显微镜(UBM)在术前术后的应用。方法 54例(54眼)拟行人工晶体植入术的无晶状体眼患者,术前行UBM检查,了解睫状沟形态、残余晶状体囊膜裂孔位置和残余囊膜的范围、大小。根据检查结果将患眼分为A、B、C组,分别行不固定、单襻固定、双襻固定的人工晶体植入术。对比观察三组患者术后1周、1个月、3个月裸眼视力(UCVA),术后矫正视力(BCVA),实际球镜度数与术前预测屈光度数的差异,人工晶体位置以及并发症发生情况。结果各组术后各时间点UCVA均优于术前(P<0.05);A组UCVA优于B组(P<0.05),B组UCVA优于C组(P<0.05)。C组术后3个月实际球镜度数大于术前预测屈光度数(P<0.05),A组、B组球镜度数术前术后差异无统计学意义(P>0.05)。84%人工晶体襻确切悬吊于睫状沟。所有人工晶体无脱位、钟摆现象。无严重手术并发症。结论 UBM术前检查有利于确定手术方式,简化手术,使人工晶体襻更准确悬吊于睫状沟。对于植入睫状沟的人工晶体,人工晶体度数应比术前预测植入囊袋内人工晶体度数适当降低。 Objective To observe the application of ultrasound biomicroscope (UBM) in secondary intraocular lens implantation in apha-kic eyes. Methods 54 cases (54 eyes) patients with aphakia underwent intraocular lens implantation. Pre - UBM examination in order to clear the ciliary sulcus structure. According to the scope and size of residual lens capsule membrane by UBM examination, aphakia eyes were divided in-to three groups, who received three different treatments, without fixation (A group) ,with one loop fixation (B group) and with both loop fixation (C group). The difference of uncorrected visual acuity (UCVA) , corrected visual acuity (BCVA) , actual spherical degree and preoperative pre-dictive diopter, intraocular lens position and complication were observed and compared among the three groups after 1 week, 1 month and 3 months. Results UCVA of each group postoperative were better than that of preoperative( P 〉0. 0 5) . UCVA in group A was better than group B , whose UCVA was better than that of group C ( P 〈0. 05 ) . The actual spherical diopter of group C was greater than that of preoperative predic-tion at 3 month after operation ( P 〈0. 05) and that was no significant difference before and after operation in group A and group B. There was 84% of the IOL loops were precisely implanted at ciliary sulcus. There were no dislocation and oscillation of all the intraocular lens. All the pa-tients did not show serious complications. Conclusion UBM is helpful to determine the surgery strategy, simplify operation. The intraocular lens loop can more accurately suspended in ciliary sulcus. For the intraocular lens implanted in ciliary sulcus, the spherical diopter of intraocular lens should be lower than that of preoperative prediction.
