摘要
目的探讨随机尿蛋白/肌酐比值(Up/Ucr)测定在基层医院慢性肾脏病(CKD)患者中的临床应用价值。方法选择2015年1月~2016年12月期间我院肾内科诊治的124例CKD 1~3期[eGFR≥30 mL/(min·1.73 m^2)]患者为研究对象,选择我院同期健康体检者35例纳入对照组,检测24 h尿蛋白定量(24 h UP)及Up/Ucr,并进行统计学分析。结果 CKD 1期、CKD 2期及CKD 3期患者Up/Ucr及24 h UP均明显高于对照组,差异有统计学意义(P<0.05);CKD 3期患者Up/Ucr、24 h Up及Scr水平均明显高于CKD 1期,差异有统计学意义(P<0.05);CKD 1期、CKD 2期患者Scr水平与对照组比较,差异无统计学意义(P>0.05)。Spearman相关分析显示,CKD 1期、CKD 2期及CKD 3期患者其Up/Ucr与24 h Up均呈正相关性(P<0.05)。结论随机尿蛋白/肌酐比值与慢性肾脏病1~3期患者的24 h尿蛋白定量具有较好的相关性,有利于基层医院CKD的早期筛查、诊断及临床治疗。
Objective To investigate the clinical value of random urinary protein/creatinine ratio(Up/Ucr) in patients with chronic kidney disease (CKD) in grassroots hospital. Methods 124 cases of CKD1-3 stage [eGFR≥30 mL/(min· 1.73 m2)] patients treated in Nephrology Department of our hospital from January 2015 to December 2016 were studied as the re- search object, the same period of 35 cases of healthy people were selected and included into the control group, 24 h urinary protein (24 h Up) and Up/Ucr were detected and analyzed statistically. Results The Up/Ucr 24 h Up in CKD 1-3 stage patients were significantly higher than those of the control group, the difference was statistically significant(P〈0.05); Up/Ucr and 24 h Up, Scr in CKD 3 stage patients were significantly higher than CKD 1 stage, the difference was statis- tically significant(P〈0.05); There was no significant difference in Scr level between CKD1, CKD2 stage patients with the control group (P〉0.05); Spearman correlation analysis showed that the Up/Ucr in the CKD1-3 stage was positive correlation with the 24 h Up(P〈0.05). Conclusion There is of good correlation between the Up/Ucr and 24 h urinary protein in patients with CKD at stage 1-3, which is conducive to the early screening, diagnosis and clinical treatment of CKD in primary hospital.
