摘要
目的探讨人乳头状瘤病毒(HPV)感染和复发性尖锐湿疣细胞皮肤免疫水平的关系,为临床预防尖锐湿疣复发提供理论依据。方法选取2015年1月-2016年6月在医院性病科门诊治疗的尖锐湿疣患者136例,分为复发组85例和未复发组51例,比较两组患者不同HPV感染亚型分布以及HPV基因型,比较两组患者外周T淋巴细胞亚群分布的差异。结果复发组患者高危型HPV分布率为69.4%显著高于未复发组31.4%,未复发组患者低危型HPV分布率为68.6%显著高于复发组30.6%,差异有统计学意义(P<0.05);复发组与未复发组患者HPV感染均以低危型HPV6,HPV11以及HPV43为主,未复发组检测到高危型基因较少,复发组患者检测到高危型基因较多;复发组患者CD3+含量(66.8±7.5%),CD_4^+含量(32.8±5.3%),CD_4^+/CD_8^+比值(0.8±0.1)均显著低于未复发组CD3+含量(75.4±8.3%),CD_4^+含量(39.2±4.8%),CD_4^+/CD_8^+比值(1.2±0.2),复发组患者CD_8^+含量(40.3±4.8%)显著高于未复发组(32.5±5.2%),差异有统计学意义(P<0.05);治疗3个月后,复发组患者CD3+(64.8±6.1%),CD_4^+含量(31.5±5.4%),CD_4^+/CD_8^+比值(0.8±0.1)均显著低于未复发组患者CD3+(75.7±8.5%),CD_4^+含量(38.1±4.6%),CD_4^+/CD_8^+比值(1.1±0.2),复发组患者CD_8^+含量(41.6±4.5%)显著高于未复发组(33.7±5.5%),差异有统计学意义(P<0.05)。结论尖锐湿疣患者HPV感染的亚基因型和患者治疗后复发存在密切的联系,尖锐湿疣患者在接受治疗前的细胞免疫水平与疾病复发之间存在密切联系,为临床预防尖锐湿疣的复发以及有效治疗尖锐湿疣的复发提供科学依据。
OBJECTIVE To investigate relationship between HPV infection and recurrent condyloma acuminatum cellular immune level,so as to provide theoretical basis for clinical prevention of recurrence of condyloma acuminatum.METHODS A total of 136 patients with condyloma acuminatum venereal treated in department of venerology in our hospital from Jan.2015 to Jun.2016 were selected,and were divided into relapsing group and non relapsing group.The different subtypes of HPV infection distribution and HPV genotypes were compared of the two groups,and the differences of the distribution of peripheral T lymphocyte subgroup of the two groups were also compared.RESULTS The high-risk type ratio of relapsing group was 69.4 %,which was significantly higher than 31.4% of non-relapsing group,and the low-risk type recurrence ratio of non relapsing group was 68.6%,which was significantly higher than 30.6% of relapsing group (P 〈0.05).Relapsing group and non relapsing group of HPV infection of patients were mainly low-risk type HPV6 and HPV11 and HPV43,the high-risk type gene detected in non-relapsing group was little,and the high risk type gene detected in relapsing group of patients was more.The contents of CD3+,CD4+ and CD4+/CD8+ were (66.8±7.5%),(32.8 ±5.3%) and (0.8±0.1) in relapsing group,which were significantly lower than (75.4±8.3%),(39.2±4.8%) and(1.2±0.2)in non-relapsing group,and the content of CD8-(40.3±4.8%) in relapse group was significantly higher than that in non-relapsing group (32.5±5.2%) (P〈0.05).After 3 months of treatment,CD3+ (64.8±6.1%),CD+(31.5 ±5.4%) and CD4 +/CD8+ ratio (0.8±0.1) in relapse group were significantly lower than (75.7±8.5%) and (38.1±4.6%) in non-relapsing group (P〈0.05),and CD8-in patients with relapse (41.6±4.5%) was significantly higher than that in non-relapse group (33.7 ± 5.5 %) (P〈0.05).CONCLUSION The genotypes of HPV infection in patients with condyloma acuminatum are closely related with the recurrence of patients after treatment,and the cellular immune level in patients with condyloma acuminatum before treatment are closely related with relapse of disease,which provides scientific basis for the prevention and treatment of recurrence of condyloma acuminatum in clinic.
