摘要
目的 探讨预约就诊对诊室血压测量值的影响.方法 连续选取2015年1月至2016年6月间于郑州大学人民医院心内科门诊就诊的符合入组条件的患者869例,按照是否预约就诊分为2组,采用t检验比较诊室内血压值与家庭自测血压值组间的差异,并采用有序多分类Logistic回归分析进行相关影响因素分析.结果 ①诊室收缩压及舒张压测量值均高于家庭自测舒张压及收缩压测量值[（141.97±19.94）mm Hg比（132.07±16.38）mm Hg,P＜0.01;（87.50±13.38）mm Hg比（83.50±12.09）mm Hg,P＜0.01].②未预约就诊组患者诊室内收缩压（OSBP）及其与家庭自测收缩压（HSBP）的差值绝对值（I△SBPI）的平均值均明显高于预约就诊组[（143.47±20.36）mm Hg比（140.03-±16.09）mm Hg,P＜0.01;（13.54±13.87）mm Hg比（8.52±12.06）mm Hg,P＜0.01].有序多分类Logistic回归分析提示,在校正年龄、性别、心率、BMI等因素后,预约就诊（OR=0.532,95％CI 0.403～0.704）与I△SBPI相关.结论 ①诊室收缩压及舒张压测量值均高于家庭自测值.②预约就诊可减少OSBP测量值与HSBP的差异.
Objective To investigate effects of appointments of outpatients on office blood pressure measurement.Methods 869 eligible patients were consecutively recruited from cardiac clinic of Henan Province People＇s Hospital from January,2015 to June,2016.Subjects were divided into 2 groups according to whether appointments or not,blood pressure data were analyzed with T-test and ordinal logistic regression analysis.Results （1）The mean systolic blood pressure （SBP） and diastolic blood pressure （DBP） measured in the office were higher than that measured at home [（141.97±19.94）mm Hg vs （132.07±16.38）mm Hg,P〈0.01;（87.50± 13.38）mm Hg vs （83.50±±12.09）mm Hg,P〈0.01].（2）The mean office systolic blood pressure （OSBP） as well as the absolute value of difference between OSBP and home measurement of systolic blood pressure（HSBP）（｜△ SBPI） of subjects without appointments were higher than that of subjects with appointments [（143.47-±20.36）mm Hg vs （140.03±16.09）mm Hg,P〈0.01;（13.54±13.87）mm Hg vs （8.52±12.06）mm Hg,P〈0.01].The results of logistic regression analysis also indicated that an appointment of outpatient （OR =0.532,95%CI 0.403-0.704） were significantly associated withl △SBPI.Conclusion （Ⅰ）SBP and DBP measured in the office are higher than that measured at home.（2）Appointment to outpatients may decrease the difference between OSBP and HSBP.
