早期立位促通联合核心肌群训练对痉挛型脑瘫患儿平衡及步行能力的影响 被引量:3

Effects of early standing position facilitation with core stability training on balance and walking ability of children with spastic cerebral palsy

摘要 目的探讨早期立位促通联合核心肌群训练对痉挛型脑瘫患儿平衡及步行能力的影响。方法将符合纳入标准的80例痉挛性脑瘫患儿随机分为4组,第1组接受常规功能训练2次(30 min/次),第2组接受常规功能训练1次(30 min/次)结合立位促通训练1次(30 min/次),第3组接受常规康复训练1次(30 min/次)结合核心肌群训练1次(30 min/次),第4组接受常规功能训练1次(30 min/次)的同时进行早期立位促通1次(15 min/次)和核心肌群训练1次(15 min/次),所有项目均为每周5次,3个月为1个疗程。治疗前后4组患儿均采用Berg平衡量表和粗大运动功能测定(GMFM-88)进行评估,根据评分结果进行组内和组间比较。结果 4组患儿与组内治疗前后平衡功能和粗大运动功能参数比较(P>0.05),方差分析发现4组患儿4种不同训练方法治疗后平衡功能、粗大运动功能分值提高差异(P<0.05),两两分析发现,第3组与第4组分值的提高差异(P>0.05)。结论早期立位促通联合核心肌群训练对痉挛型脑瘫患儿平衡及步行能力有明显的促进作用,有利于改善异常步态,提高患儿生活自理能力。 Objective To explore the effects of early standing position facilitation with core stability training on the balance and walking ability of children with spastic cerebral palsy. Methods A total of 80 children with spastic cerebral palsy conformed to the standard were randomly divided into four groups. The first group received the routine function training twice(30 min/time), the second group received the routine function training once(30 min/time) combined with the standing position facilitation once(30 min/time), the third group received the routine rehabilitation training once(30 min/time) combined with the core stability training once(30 min/time), and the fourth group received the routine function training once(30 min/time) combined with the early standing position facilitation once(15 min/time) and core stability training once(15 min/time). All items were five times a week, and 3 months were a course of the treatment. All the four groups were assessed with the Berg balance scale and gross motor function measure(GMFM-88) before and after the treatment. According to the grading results, the effects were compared within the group and between the groups. Results There were no significant differences among the four groups before and after treatment in the balance function and gross motor function parameters of the children(P〉0.05). The variance analysis showed that the scores of the balance function and gross motor function of the children improved in the four groups after the four different trainings, and the improved degrees had a statistically significant difference among the four groups(P〈0.05). The two by two analysis showed that the scores improved obviously in the third and the fourth groups, and there was no significant difference between the two groups(P〉0.05). Conclusion The early standing position facilitation with core stability training has obvious effects on the balance and walking ability of children with spastic cerebral palsy. It is good for the children to improve their abnormal gaits and self-care abilities of daily living.
