摘要
目的应用《微创注射填充安全操作管理规范及并发症防治措施指南(2015版)》指导透明质酸注射填充后并发急性血运障碍患者的救治。方法自2015年6月至2016年12月,对35例面部透明质酸注射填充后并发急性血运障碍的患者,按照指南流程酌情给予切开减压、高压氧、抗炎、溶栓、抗凝、扩血管等对症治疗,最大程度地挽救缺血过渡区。并观察记录局部皮肤改善情况,对救治效果进行评估。结果 31例患者未遗留皮肤缺损及创面,2例患者仅有轻度色素沉着,1例患者单侧鼻翼轻度缺损,1例患者瘢痕愈合。结论眉间、鼻唇沟及鼻背均为注射填充的危险区,极易发生血管内注射,出现严重的并发症。《微创注射填充安全操作管理规范及并发症防治措施指南(2015版)》能系统、规范、全面、有效地指导局部血运障碍的救治,值得临床参考。
Objective To explore the guiding function of the management practices for safe injection operations and guidelines for prevention and treatment of complications in hemodynamic disorder rescue after hyaluronic acid injection.Methods From June 2015 to December 2016,35 patients with hemodynamic disorder after hyaluronic acid injection were collected according to the standard process.Local skin improvement was observed and the therapeutic effect was evaluated.After emergency admission,symptomatic treatment such as open decompression,hyperbaric oxygen therapy,anti-inflammatory,thrombolysis,anticoagulation and vasodilation were conducted according to the management practices for safe injection operations and guidelines for prevention and treatment of complications.care was taken to save the ischemic transition zone.Results Thirty-one patients were completely healed without occurrence of skin defect or wound,2 patients were with mild pigmentation,1 patient was with mild unilateral nasal alardefectsand there was 1 case of scar healing.Conclusions Complications are inclined to occurin the glabella,dorsum nasi and nasolabial fold,which are danger zones for facial injection.The management practices for safe injection operations and guidelines for prevention and treatment of complications can fully and effectively guide the rescue of hemodynamic disorder.It should be recommended for clinical reference.
