防压疮气垫床压力选择的实证研究与分析 被引量:13

摘要 目的探讨临床防压疮气垫床的最佳压力。方法在改装设备测量气垫床压力和骶尾部压力的基础上,采用重复测量方法,选择76名健康志愿者仰卧于气垫床上,气垫床压力由充满逐渐降低,每降低5mmHg记录1次骶尾部压力;使用混合效应二次曲线模型分析气垫床压力与骶尾部压力的关系,通过固定效应确定最佳的气垫床压力,并根据个体随机效应分析气垫床压力的最佳范围。结果气垫床压力和骶尾部压力呈二次项分布,可以采用混合效应二次曲线模型进行分析;通过固定效应分析可得气垫床的最佳压力为24.55mmHg,根据个体随机效应分析可得气垫床的压力范围在20.23~29.40mmHg时,人体骶尾部所受的压力最低,与其他范围相比差异具有统计学意义(均P<0.01)。结论气垫床最佳压力范围为20.23~29.40mmHg。 Objective To explore the best pressure level for clinical use of bedsore-preventing air mattress. Methods On the basis of measuring the pressures of air mattress and sacrococcygeal region by refitted equipments, repeated measurement method was used in this study, in which 76 healthy volunteers were chosen to lie on a fully inflated air mattress which was gradually deflated and the sacrococcygeal pressure was recorded at each 5-mmHg reduction. The relationship between air mattress pressure and sacrococcygeal pressure was analyzed by using mixed effect of quadratic curve model, then the best pressure value and its optimum range were determined through fixed effect, Results Mixed effect of quadratic curve model could be used to analyze the relationship between the: air mattress pressure and saerococcygeal pressure while they were presenting quadratic distribution; the best pressure of air mattress was 24.55 mmHg by fixed effect analysis, the sacrococcygeal pressure was the lowest while the air mattress pressure was 20.23-29.40 mmHg by individual random effects analysis; there were statistically significant differences compared with other ran ges. Conclusion The optimum range of air mattress was 20.23-29.40 mmHg.
