异丙酚复合瑞芬太尼靶控输注对脑肿瘤手术患者脑氧供需平衡的影响 被引量:7

Effect of propofol combined with remifentanil target controlled infusion on brain tumor surgery

摘要 目的比较脑肿瘤患者手术麻醉过程中应用异丙酚复合瑞芬太尼与七氟醚复合瑞芬太尼的效果。方法采用随机软件SAS 8.3将2015年6月至2016年6月于本院治疗的80例脑肿瘤患者随机分为观察组和对照组,每组各40例。观察组患者采用异丙酚复合瑞芬太尼靶控输注,对照组患者采用七氟醚复合瑞芬太尼麻醉。比较两组患者麻醉期不同时间点的血流动力学、手术麻醉时间、颈静脉球血氧饱和度(SjvO_2)、脑动静脉血氧含量差(Da-SjvO_2)。结果两组患者血流动力学和手术麻醉时间比较差异均无显著性(P>0.05)。观察组患者SjvO_2显著低于对照组(P<0.01),但Da-SjvO_2显著高于对照组(P<0.01)。结论异丙酚复合瑞芬太尼与七氟醚复合瑞芬太尼均可安全用于脑肿瘤手术患者的麻醉,异丙酚复合瑞芬太尼麻醉可以维持稳定的脑氧供需平衡,而七氟醚联合瑞芬太尼可以缩短患者术后苏醒时间,还可改善脑氧供状态。 Objective To compare the anesthesia effect of propofol remifentanil and sevoflurane remifentanil anesthesia brain tumor patients during the operation. Method 80 patients with brain tumor were randomly divided into observation group and control group, each group 40 patients by using the random software SAS 8.3 from June 2015 to June 2016. The patients in observation group were given propofol and remifentanil target controlled infusion, while patients in control group treated with sevoflurane combined with remifentanil anesthesia. The outcome measurements included hemodynamics, anesthesia time, jugular bulb oxygen saturation (SjvO2) and cerebral arteriovenous oxygen content difference (Da-SjvO2) at different time points during anesthesia. Result No significant differences of hemodynamics and anesthesia time were found between the two groups (P 〉 0.05). The SjvO2 in observation group was significantly lower than that in control group (P 〈 0.01). However, the Da-SjvO2 in observation group was significantly higher than that in control group (P 〈 0.01). Conclusion Both propofol and remifentanil target controlled infusion, and sevoflurane combined with remifentanil anesthesia can be safely applied to the brain tumor surgery. However, the propofol and remifentanil can balance the oxygen supply, while sevoflurane combined with remifentanil anesthesia can make the patients recovery from the surgery more quickly, and enhance the oxygen supply for brain.
