摘要
目的 评价栝楼薤白半夏汤加味结合西医常规疗法治疗气滞血瘀型冠心病心绞痛的疗效.方法 将符合入选标准的气滞血瘀型冠心病心绞痛患者62例按随机数字表法分为2组,每组31例.对照组服用硝酸异山梨醇酯片、阿司匹林肠溶片及盐酸阿替洛尔片;观察组在对照组基础上加用栝楼薤白半夏汤加味.2组均治疗30 d.观察患者症状、体征及心电图变化,采用免疫放射法测定B型脑钠肽（B-type natriuretic peptide,BNP）,免疫比浊法测定血清CRP,评价临床疗效.结果 观察组临床总有效率为96.8%（30/31）、对照组为83.9%（26/31）,2组比较差异有统计学意义（χ2=6.287,P=0.023）;观察组证候疗效总有效率为87.1%（27/31）、对照组为71.0%（22/31）,2组比较差异有统计学意义（χ2=6.124,P=0.024）;观察组心电图疗效总有效率为96.8%（30/31）、对照组为83.9%（26/31）,2组比较差异有统计学意义（χ2=6.287,P=0.023）.治疗后,观察组血清CRP[（4.22±1.87）mg/L比（7.21±1.88）mg/L,t=5.643]、BNP[（31.87±12.58）mg/L比（59.28±13.21）mg/L,t=6.769]水平及血浆黏度[（1.32±0.02）mPa？s比（1.57±0.04）mPa？s,t=5.769]低于对照组（P〈0.01）.结论 栝楼薤白半夏汤加味结合西医常规疗法可有效改善气滞血瘀型冠心病心绞痛患者临床症状,降低血清CRP、BNP水平及血液黏稠度.
Objective To evaluatethe effect of Gualou-Xiebai-Banxia decoction combined with convetional therapy for the patients with coronary heart disease angina pectoris （stagnancy of qi and blood stasis）.Methods A total of 62 patients with angina pectoris （stagnancy of qi and blood stasis）, randomly divided into 2 groups according to the random number table. The control group was treated with isosorbide dinitrate tablets, aspirin enteric-coated tablets and atenolol hydrochloride tablets; the observation group was treated withGualou-Xiebai-Banxia decoction on the basis of the control group. Both groups were treated for 30 days. The changes of symptoms, signs and electrocardiogram were observed. The B-type brain natriuretic peptide （BNP） and surum CRP was measured by immunoradiometric method. The clinical curative effect was evaluated. ResultsThe total effective rate was 96.8% （30/31） in the observation group and 83.9% （26/31） in the control group. There was significant difference between the two groups （χ2=6.287,P=0.023）. The total effective rate of TCM patterns was 87.1% （27/31） in the observation group and 71.0% （22/31） in the control group. There was significant difference between the two groups. The total effective rate of ECG was 96.8%（30/31） in the observation group and 83.9% （26/31） in the control group. There was significant difference between the two groups （χ2=6.287,P=0.023）. After treatment, the serum levels of CRP （4.22 ± 1.87 mg/Lvs. 7.21 ± 1.88 mg/L,t=5.643）, BNP （31.87 ± 12.58 mg/Lvs. 59.28 ± 13.21 mg/L,t=6.769） and plasma viscosity （1.32 ± 0.02 mPa-svs. 1.57 ± 0.04 mPa-s,t=5.769） in the observation group were significantly lower than those in the control group （P〈0.01）.Conclusions The Gualou-Xiebai-Banxia decoction combined convetional therapy could improve the clinical symptoms of the patients with coronary heart disease angina pectoris （stagnancy of qi and blood stasis） , could also reduce the serum CRP, BNP levels and blood viscosity.
