摘要
目的：对比经超声引导下微创旋切术与传统切除术治疗腋窝副乳腺的疗效,评价微创旋切技术在切除腋窝副乳腺治疗中的应用价值。方法：148例乳腺外科女性患者分为旋切组（106例）和传统组（48例）,分别应用超声引导下的微创旋切术和传统切除术进行治疗,观察术中出血量,肌肉、血管神经损伤情况;观察术后感染、患肢肿胀、疼痛及功能状态;随访3~18个月,评价两种手术方式的治疗效果、患者满意度及治疗费用。结果：在肌肉损伤/破坏方面,旋切组的发生率低于传统组（P＜0.05）;在术中出血方面,旋切组的发生率低于传统组（P＜0.01）;两组手术在手术效果方面的比较,超声引导下微创旋切术切除副乳腺较传统切除术在切口大小和疤痕长度等手术效果指标上,两者有显著性统计学差异（P＜0.01）;旋切组和传统组的治愈率相近,但是前者在手术时间、术后恢复、住院时间、手术创伤、术中术后并发症、住院费用等效果上,均显著优于传统切除术。结论：超声引导下的麦默通微创旋切术切除腋窝副乳腺,具有微创、美容、提高患者术后生活质量高和产生更多社会效益的优点,有较高的应用价值,值得在临床上广泛推广。
Objective： To compare the therapeutic effect of ultrasound guided minimally invasive ex- cision with traditional excision in treatment of axillary accessary breast, and to evaluate its clinical ap- plication value. Methods： One hundred and forty-eight woman patients with axillary accessary breast from March 2015 to March 2016 were divided into two groups. Group A （u = 106） aeeepted ultrasound guided minimally invasive excision collectively. Group B （u = 42） , was treated with traditional exci- sion. The bleeding amount, damage on muscle, blood vessel, and nerve during the operation and post- operation infection, swelling, pain, and function of the limb were observed and compared, and follow- up visit was conducted for 3 - 18 months. Treatment effects, patient satisfaction and curing costs were evaluated. Results： The incidence of surgieal trauma and operation bleeding amount were lower in group A than in group B （P 〈 0.05, and P 〈 0. 01 ）. The operation effects, based on standards of in- cision size and scar length, were better in group A than in group B （P 〈 0.01 ）. Although the cure rates in the 2 groups were very close, the general effects based on operative time, postoperative recov- ery time, surgical trauma, postoperative complications and hospitalization costs were better in group A than in group B. Conclusion： Ultrasound guided mammotome minimally invasive excision under the armpits of mammary gland, brings high life quality to patients, which has high application value and is worth to promote in clinic.
