摘要
目的探讨急诊腹腔镜疝囊高位结扎术治疗小儿腹股沟嵌顿性斜疝的临床应用价值。方法选取我院2015年6月至2016年6月间收治的小儿腹股沟嵌顿性斜疝患儿80例,通过随机数表法将患儿分为常规组及实验组各40例。给予常规组传统开放手术治疗,给予实验组腹腔镜疝囊高位结扎术治疗。比较两组患儿手术情况指标,包括手术时间、术中出血量、离床活动时间、住院时间和VAS评分。结果实验组的出血量、离床活动时间和住院时间均少于常规组,差异均具有统计学意义(P<0.05)。实验组的术后24 h及72 h的VAS评分均低于常规组,差异均具有统计学意义(P<0.05)。实验组的并发症发生率显著低于常规组,差异具有统计学意义(P<0.05)。结论采用腹腔镜疝囊高位结扎术治疗小儿腹股沟嵌顿性斜疝,创伤小,出血量少,患儿恢复快,住院时间短,并发症较少,值得推广。
Objective To explore the clinical application value of laparoscopic high ligation in treatment of incarcerated inguinal hernial sac in infants. Methods 80 cases of children with incarcerated inguinal hernial sac admitted to our hospital from June 2015 to June 2016 were selected and randomly divided into conventional group and experimental group, with 40 cases in each group. The conventional group received traditional open surgery, while the experimental group received laparoscopic high ligation. The operation indexes of children, including operation time, intraoperative blood loss, ambulation time, hospitalization time and VAS score, were compared between two groups. Results The blood loss, ambulation time and hospitalization time of experimental group were significantly less than those of conventional group, with statistical difference （P 〈0.05）. The VAS scores 72 h and 24 h after surgery of experimental group were significantly lower than those of conventional group, with statistical differences （P 〈0.05）. The incidence of complications of experimental group was lower than that of conventional group, with statistical difference （P 〈0.05）. Conclusions Laparoscopic high ligation in the treatment of children with incarcerated inguinal hernial sac has less trauma and blood loss, fast recovery, shorter hospitalization time and less complications, which is worthy of promotion.
