左氧氟沙星使用与细菌耐药率相关性分析 被引量:2

摘要 目的:统计某三甲医院2009-2015年左氧氟沙星使用与10种主要细菌耐药率之间的相关性,为降低细菌耐药性和合理使用抗菌药提供参考依据。方法:采用回顾性调查法对2009-2015年左氧氟沙星使用及细菌耐药率情况进行统计分析抗菌药物消耗量和细菌耐药率之间的相关性。结果:左氧氟沙星消耗量从2009年11598.20DDD到2015年8023.60DDD,下降了30.82%。左氧氟沙星使用与鲍曼不动杆菌耐药率无相关性,与溶血性葡萄球菌(r=-0.64)和屎肠球菌(r=-0.64)耐药率存在负相关,除此之外与常见细菌耐药率存在正相关,其中与铜绿假单胞菌(r=0.79,P<0.05)及粪肠球菌(r=0.93,P<0.01)耐药率存在显著正相关。结论:某院左氧氟沙星抗菌药物使用频度呈下降趋势;除鲍曼不动杆菌、溶血性葡萄球菌和屎肠球菌外,左氧氟沙星的使用与常见病原菌的耐药率呈正相关;提高左氧氟沙星使用频度可能导致某些细菌耐药率增高。 Objective: To statistically analyze the utilization of levofloxacin and the resistance rate of 10 primary bacteria in a hospital during 2009-2015,and study the relationship between them,so as to reduce bacterial resistance and to provide references for clinical rational use of antimicrobial drugs. Methods: Retrospective investigation method was used to analyze the statistic correlation between resistance rate of bacteria and antimicrobial consumption. Results: The consumption of levofloxacin decreased from 11598. 20 DDD in 2009 to 8023. 60 DDD in 2015,with the drop rate of 30. 82%. There was no correlation between consumption of levofloxacin and resistant rate of Acinetobacter baumannii. Negative correlation existed between the use of levofloxacin and the resistance rate of Staphylococcus hemolytic( r =-0. 64) and Enterococcus faecium( r =-0. 64). Positive correlation was found with the resistance rate of common bacteria,especially a significant positive correlation with Pseudomonas aeruginosa( r = 0. 79,P〈0. 05) and Enterococcus faecalis( r = 0. 93,P〈0. 01). Conclusion: The frequency of levofloxacin usage showed a declining trend in the hospital. Except for Acinetobacter baumannii,Staphylococcus hemolytic and Enterococcus faecium,the drug use of levofloxacin and the resistance rate of mean pathogenic bacteria are positively correlated. To increase the frequency of levofloxacin use may result in the rise of resistant rate of certain bacteria.
