美托洛尔联合曲美他嗪对冠心病心力衰竭患者炎性反应、T细胞亚群及心功能的影响研究 被引量:124

摘要 目的探讨美托洛尔联合曲美他嗪对冠心病心力衰竭患者炎性反应、T细胞亚群及心功能的影响。方法选取2013年5月—2015年10月铜川矿务局中心医院收治的冠心病心力衰竭患者90例,按随机数字表法分为对照组与观察组,每组45例。两组患者均予以常规抗心力衰竭治疗,同时对照组患者予以美托洛尔治疗,观察者患者在对照组基础上加用曲美他嗪治疗;两组患者均连续治疗6个月。比较两组患者治疗前后心功能指标〔左心室射血分数(LVEF)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)〕、血浆C反应蛋白(CRP)和脑利钠肽(BNP)水平及T细胞亚群(CD_4^+ 细胞分数、CD_8^+细胞分数、CD_4^+ /CD_8^+细胞比值)。结果治疗前两组患者LVEF、LVESD、LVEDD比较,差异无统计学意义(P>0.05);治疗后观察组患者LVEF高于对照组,LVEDD、LVESD低于对照组(P<0.05)。治疗前两组患者血浆CRP、BNP水平比较,差异无统计学意义(P>0.05);治疗后观察组患者血浆CRP、BNP水平低于对照组(P<0.05)。治疗前两组患者CD_4^+ 细胞分数、CD_8^+细胞分数、CD_4^+ /CD_8^+细胞比值比较,差异无统计学意义(P>0.05);治疗后观察组患者CD_4^+ 细胞分数、CD_4^+/CD_8^+细胞比值高于对照组(P<0.05),两组患者CD_8^+细胞分数比较,差异无统计学意义(P>0.05)。结论美托洛尔联合曲美他嗪可有效减轻冠心病心力衰竭患者炎性反应,调节T细胞亚群,改善心功能。 Objective To investigate the impact of metoprolol combined with trimetazidine on inflammatory reaction, T - lymphocyte subsets and cardiac function of coronary heart disease patients complicated with heart failure. Methods A total of 90 coronary heart disease patients complicated with heart failure were selected in the Central Hospital of Tongchuan Mining Bureau from May 2013 to October 2015,and they were divided into control group and observation group according to random number table,each of 45 cases. Based on conventional treatment for heart failure, patients of control group were given metoprolol,while patients of observation group were given metoprolol combined with trimetazidine;both groups continuously treated for 6 months. Index of cardiac function(including LVEF,LVESD and LVEDD),plasma levels of CRP and BNP,and T- lymphocyte subsets(including CD +4 cell percentage,CD +8 cell percentage and CD +4 / CD +8 cell ratio)were compared between the two groups before and after treatment. Results No statistically significantly differences of LVEF,LVESD or LVEDD was found between the two groups before treatment( P > 0. 05 );after treatment,LVEF of observation group was statistically significantly higher than that of control group,while LVEDD and LVESD of observation group were statistically significantly lower than those of control group(P < 0. 05). No statistically significant differences of plasma level of CRP or BNP was found between the two groups before treatment( P > 0. 05),while plasma levels of CRP and BNP of observation group were statistically significantly lower than those of control group(P < 0. 05). No statistically significant differences of CD +4 cell percentage,CD +8&nbsp;cell percentage or CD +4 / CD +8 cell ratio was found between the two groups before treatment,nor was CD +8 cell percentage between the two groups after treatment( P > 0. 05),while CD +4 cell percentage and CD +4 / CD +8 cell ratio of observation group were statistically significantly higher than those of control group after treatment(P < 0. 05). Conclusion Metoprolol combined with trimetazidine can effectively relieve the inflammatory reaction,adjust the T - lymphocyte subsets and improve the cardiac function of coronary heart disease patients complicated with heart failure.
