摘要
目的 探讨带锁髓内钉内固定和动力加压钢板内固定对股骨干骨折后骨不连的治疗效果。方法 选取2011年2月~2015年1月收治的76例股骨干骨折后骨不连患者,按随机数字表法将其分成甲组和乙组,每组38例。甲组采用带锁髓内钉内固定术治疗,乙组采用动力加压钢板内固定进行治疗。比较两组手术时间、出血量、切口长度、术后引流量、近期治疗效果、骨折愈合时间、随访期间感染发生率及治疗前后膝关节功能。结果 两组手术时间及切口长度比较差异无统计学意义（P〉0.05）;甲组出血量及术后引流量均明显少于乙组（P〈0.05）;甲组骨折愈合时间短于乙组（P〈0.05）;甲组感染发生率明显低于乙组（P〈0.05）;甲组治疗的总有效率明显高于乙组（P〈0.05）;治疗前两组膝关节功能得分比较差异无统计学意义（P〉0.05）;治疗后甲组膝关节功能得分明显高于乙组（P〈0.05）。结论 与动力加压钢板内固定比较,带锁髓内钉内固定治疗股骨干骨折后骨不连近、远期疗效都更好,且患者恢复更快,感染发生率更低。
Objective To compare the therapeutic effect of interlocking intramedullary nail for internal fixation and dynamic compression plate for internal fixation on treating bone ununion in fracture of femoral shaft.Methods From February 2011 to January 2015,76 patients with bone ununion in fracture of femoral shaft admitted into our hospital were selected.According to a random number table,they were evenly divided into group A and group B.In group A, interlocking intramedullary nail for internal fixation was applied,while in group B,dynamic compression plate for internal fixation was adopted.The operation time,amount of bleeding,volume of drainage after surgery,length of incision, short-term therapeutic effect,healing time of bone fracture,incidence of infection during follow-up visit,and knee joint function before and after treatment were compared between the two groups.Results There was no great difference in operation time or length of incision between two groups （P〉0.05）.The amount of bleeding and volume of drainage after surgery in group A was obviously less than that in group B respectively （P〈0.05）.In group A,healing time of bone fracture was shorter in comparison with that in group B （P〈O.O5）.The incidence of infection in group A was lower compared with that in group B （P〈0.05）.The total effective rate in group A was much higher than that in group B （P〈 0.05）.There was no significant difference in score of knee joint function before treatment in both groups （P〉0.05）. However,after therapy,the score of knee joint function in group A was remarkably higher than that in group B （P〈0.05）. Conclusion Compared with dynamic compression plate for internal fixation,interlocking intramedullary nail for internal fixation can obtain better short/long-term effects on treating bone ununion in fracture of femoral shaft with recovery sooner and lower infection incidence.
