阴道微生物群落结构与复发型外阴阴道念珠菌病的相关性研究 被引量:2

摘要 目的研究阴道微生物群落结构与复发型外阴阴道念珠菌病的相关性。方法选择复发型外阴阴道念珠菌病女性患者110例设为观察组,同时选择110例健康体检女性设为对照组,比较两组阴道pH值和Nugent革兰染色评分,两组的菌群多样性,并且对两组阴道菌群优势菌进行对比。结果观察组的阴道pH值和Nugent革兰染色评分高于对照组的阴道pH值和Nugent革兰染色评分,差异具有统计学意义(P<0.05);观察组菌群多样性主要集中在Ⅰ-Ⅱ级,Ⅲ和Ⅳ级比较少见,对照组菌群多样性主要集中在Ⅰ级,Ⅱ级比较少见,观察组的菌群多样性Ⅰ级低于对照组、Ⅱ级高于对照组,差异有统计学意义(P<0.05);观察组和对照组在阴道菌群优势菌比例方面比较差异有统计学意义(P<0.05)。结论外阴阴道念珠菌病患者的阴道微生物群落结构较正常人群会发生明显变化,主要体现在阴道pH值升高、菌群多样性集中在Ⅰ-Ⅱ级、阴道菌群优势菌改变,造成阴道生物屏障严重被削弱。 [Objective] To research the dependency of vaginal microbial community structure and recurrent vulvovaginal candidiasis. [Methods] Totally 110 patients with recurrent vulvovaginal candidiasis were selected as the observation group. Meanwhile, 1 l0 healthy women were selected as the control group. The vaginal pH value and Nugent score, bacterial diversity and dominant bacteria of the vaginal flora of the two groups were compared. [Results] The pH value and Nugent score of the observation group were significantly greater than those of the control group, and the differences were statistically significant (P 〈0.05). The bacterial diversity of the observation group was mainly focused on the level I-II, relatively infrequent on the level III and IV. The bacterial diversity of the control group was mainly focused on the level I, relatively infrequent on the level II. The bacterial diversity of level I of the observation group was less than the control group, and that of level II was greater than the control group, and the differences were statistically significant (P 〈0.05). The difference in the proportion of the dominant bacteria between the two groups was statistically significant (P 〈0.05). [Conclusion] Compared with normal people, vaginal microbial community structure of patients with vulvovaginal candidiasis will be significantly changed. It shows that the pH value is increased, the bacterial diversity is focused on level I-II, and the dominant bacteria of vaginal flora is changed, which will severely weaken the vaginal biological barriers.
