摘要
目的探讨声触诊组织定量(VTQ)技术在子宫内膜疾病诊断及鉴别诊断中的价值。方法应用VTQ技术测量84例子宫内膜病变患者(病变组)及34例子宫内膜正常女性(健康对照组)的剪切波速度(SWV),结合病理检查结果,分析不同病理类型子宫内膜病变的差异。结果病例组SWV值为(2.68±0.88)m/s,健康对照组SWV值为(0.96±0.38)m/s,差异有统计学意义(P<0.05)。病例组中良性病变74例,包括23例子宫内膜增生、26例子宫内膜息肉及25例子宫黏膜下肌瘤,SWV值分别为(1.87±0.44)m/s、(2.09±0.38)m/s及(3.11±0.40)m/s;恶性病变10例均为子宫内膜癌,SWV值为(3.89±0.42)m/s,良性病变的SWV值低于恶性病变,差异有统计学意义(P<0.05)。病例组4种子宫内膜病变的SWV值均明显高于健康对照组,差异均有统计学意义(P<0.05)。不同良性病变间两两比较,子宫内膜息肉、子宫内膜增生的SWV值低于子宫黏膜下肌瘤(P<0.05),子宫内膜息肉与子宫内膜增生SWV值比较差异无统计学意义。结论 VTQ技术可以通过测量SWV值定量评价不同组织的弹性及硬度,对诊断及鉴别诊断子宫内膜病变有一定价值。
Objective To explore the value of virtual touch tissue quantification(VTQ)technique in the diagnosis and differential diagnosis of endometrial disease. Methods The endometrium of 84 patients with endometrial disease and 34 healthy women were examined by VTQ, the shear wave velocity(SWV) was measured and recorded. The findings were in contrast to pathological results.The differences in different pathological types were analyzed.Results The SWV of 84 patients with endometrial lesions and 34 healthy women were(2.68 ± 0.88)m/s and(0.96 ± 0.38)m/s(P〈0.05). 74 cases with benign lesions and the SWV was(2.37 ± 0.67)m/s, 10 cases were endometrial cancer and the SWV was(3.89 ± 0.42)m/s. The patients with benign lesions included 23 cases of endometrial hyperplasia, 26 cases of endometrial polyps and 25 cases of submucosal uterine fibroid, the corresponding SWVs were(1.87 ± 0.44)m/s,(2.09 ± 0.38)m/s and(3.11 ± 0.40)m/s, respectively. The SWV of patients with benign lesions was lower than that of patients with endometrial cancer(P〈0.05). A statistically significant difference was found in SWV between submucosal uterine fibroid and endometrial hyperplasia,submucosal uterine fibroids and endometrial polyps(P〈0. 05),while there was no significant difference in SWV between endometrial hyperplasia and endometrial polyps.Conclusion VTQ technique can quantitatively evaluate tissue elasticity and hardness, which has a certain value for the diagnosis and differential diagnosis of endometrial disease.
