摘要
目的采用Meta分析方法比较锁骨钩钢板和锁骨远端锁定钢板内固定治疗NeerⅡ型锁骨远端骨折术后并发症和患肢功能恢复情况。方法计算机检索Pub Med、中国知网、万方数据库,对所有已发表的文献进行手工检索,对参考文献进行查阅,无语种限制检索文献。采用Cochrane 5.1.1手册评价纳入文献质量。观察术后3、6个月Constant-Murley肩关节功能评分及术后并发症(肩关节疼痛、肩关节外展受限、骨折延迟愈合、肩峰撞击)。结果纳入符合标准的9篇文献,共计379例。Meta分析结果表明,术后3、6个月锁骨远端锁定钢板组肩关节功能改善程度优于锁骨钩钢板组。锁骨远端锁定钢板组术后肩关节疼痛例数、肩关节外展受限例数少于锁骨钩钢板组,差异有统计学意义(P<0.05)。2组在术后骨折延迟愈合及术后出现肩峰撞击方面差异无统计学意义(P>0.05)。结论与锁骨钩钢板内固定相比,锁骨远端锁定钢板内固定治疗NeerⅡ型锁骨远端骨折术后肩关节功能恢复较好,且术后较少出现疼痛及肩关节外展受限等并发症。
Objective To compare the postoperative complications and functional restoration using clavicle hook plate and distal clavicle locking plate to treat Neer type Ⅱ distal clavicle fractures with Meta analysis. Methods By using the computer, Pub Med, CNKI and Wanfang database were searched to retrieve all the published literature and reference literature,without language restrictions on literature retrieval. The Cochrane 5.1.1 manual was used to evaluate the literature quality. The Constant-Murley shoulder joint function score in 3 and 6 months after the operation, and the postoperative complications after the operation（shoulder joint pain, abduction restriction, fracture delay healing, subacromial impingement） were observed.Results There were 9 papers in accordance with the criteria, and total of 379 cases were evaluated. Meta analysis showed that the improvement of the Constant-Murley shoulder joint function score in distal locking plate group was better than that in the clavicle hook plate group in 3 and 6 months after the operation. The number of shoulder joint pain and shoulder joint abduction limited cases were less in distal locking plate group, and the difference was statistically significant（ P〈0.05）. There was no statistically significant differences in fracture delay healing and subacromial impingement between 2 groups（ P〈0.05）.Conclusion Compared with clavicular hook plate, the distal clavicle locking plate for Neer type II distal clavicle fracture has better shoulder function recovery with fewer complications of pain and the abduction restriction.
