摘要
目的:探讨一次性根管预备热牙胶即刻充填治疗慢性根尖周炎的临床疗效。方法:选择60例患者67颗患牙,采用抛掷硬币的方法分为观察组30例34颗患牙和对照组30例33颗患牙,观察组采用一次性根管预备热牙胶即刻充填治疗,对照组采用冷牙胶充填法治疗,比较二者根管充填时间、术后疼痛发生率、根管填充效果及治疗效果。结果:观察组患者根管充填时间及术后疼痛发生率均低于对照组,差异有统计学意义(P<0.05)。治疗1周后,行X线片检查评价根管填充效果,观察组优于对照组,差异有统计学意义(P<0.05)。术后3个月及术后1年,两组患者临床治疗效果相似,差异无统计学意义(P>0.05);术后2年,观察组患者临床治疗效果优于对照组,差异有统计学意义(P<0.05)。结论:一次性根管预备热牙胶即刻充填治疗慢性根尖周炎缩短根管充填时间,降低术后疼痛发生率,远期临床效果好。
Objective： To investigate the clinical efficacy of one-time instant root canal filling with prepared warm gutta-percha in treatment of chronic periapical periodontitis. Methods： A total of 60 patients with67 affected teeth were selected,and randomly divided into observation group（ 30 patients with 34 affected teeth） and control group（ 30 patients with 33 affected teeth）. The observation group was treated with one-time instant root canal filling with prepared warm gutta- percha,while the control group treated with filling with cold gutta- percha,and the root canal filling times,incidences of postoperative pain,root canal filling effects,and efficacies were compared between the two groups. Results： The patients of the observation group had significantly shorter root canal filling time and significantly lower incidence of postoperative pain than the control group（ P〈0.05）. At 1 week after the treatment,the root canal filling effects were evaluated based on X-ray films,and the observation group had significantly better effects than the control group（ P〈0. 05）. 3months and one year after the treatment,the patients of the two groups had insignificantly different clinical efficacies（ P〈0.05）; and at 2 years after the treatments,the observation group had significantly better clinical efficacies than the control group（ P〈0. 05）. Conclusion： One- time instant root canal filling with prepared warm gutta-percha in treatment of chronic periapical periodontitis can shorten the root canal filling time and reduce the incidence of postoperative pain,and has good long-term clinical efficacy.
