摘要
目的评估患者治疗教育( therapeutic patient education, TPE)在儿童特应性皮炎(atopic dermatitis,AD)长期治疗管理中的作用,了解TPE对AD疾病严重程度、复发频率以及对患者生活质量的改善作用;评估以“AD教育课堂”为主的AD患者治疗教育形式的可行性。方法北京儿童医院皮肤科门诊诊治的298例2~12岁AD患儿按照随机分组原则分为实验组(inter0vention group,IG)和对照组(control group,CG),采用“AD教育课堂”的形式对实验组患儿及其护理人员实施患者教育,随访期限6个月,分别在实验开始时(TO)、3个月(T1)、6个月(T2)对患儿进行SCORAD评分评估并填写儿童及幼儿皮肤病生活质量指数调查问卷、皮肤护理.润肤剂使用认知调查问卷等,以评估TPE在AD长期治疗管理中的作用,判断此教育形式的有效性及可行性。结果实验组患儿的SCORAD评分、生活质量(quality of life,QoL)均有显著改善,护理人员对皮肤护理重要性的认知程度较前增加,实验组评估结果明显优于对照组,差异有统计学意义。结论TPE在AD患儿长期治疗管理中发挥着重要作用,本研究采取的以“AD教育课堂”为主要形式的患者治疗教育形式是可行的,其可积极推广,但需不断完善。
Objective To determine the effects of age-related, structured educational programmes on the long-team management of atopic dermatitis (AD) in childhood; To evaluate the feasibility of " AD therapeutic educational class". Methods Two hundred and ninety-eight patients with AD from Beijing Children's Hospital, aged 2-12 years, derided into 2 groups : intervention group ( IG), control group (CG). Using the "AD therapeutic educational class" model to send massages to participators in IG. Complete data sets at the 6-month follow-up were available. The severity of the AD measured with the Scoring Atopic Dermatitis (SCORAD) ], Dermatology Life Quality Index, Skin Care-Questionnaires of using Emollients. Results The intervention group showed greater improvement in SCORAD, QoL, and care-givers show a great improvement in skin care. Conclusion The training program, which was tested in our program, is feasible; and plays an important role in the long treatment in AD ; it is worth of being generalized. And we can try other forms too.
