摘要
目的观察依那普利治疗老年脑血栓患者的临床效果及其对内皮素-1(ET-1)、超氧化物歧化酶(SOD)水平的影响。方法选取2014年10月一2015年10月济宁医学院附属医院神经内科住院治疗的老年脑血栓患者96例作为研究对象,按随机数字表法分为观察组48例和对照组48例。2组患者均常规控制血糖、血脂、血压;对照组在常规治疗基础上给予右旋糖酐注射液500 ml静脉滴注,1次/d;丹参川芎嗪注射液10 m溶于0.9%NaCI溶液250 ml中静脉滴注,1次/d;同时口服烟酸50 mg,3次/d。观察组在对照组基础上口服复方依那普利片5 mg,2次/d。2组患者1周为1疗程,连续治疗2周。比较2组患者临床效果,观察血压、神经功能缺损评分、ET-1、SOD含量及血液流变学变化情况。结果治疗后观察组总有效率高于对照组,差异有统计学意义(91.7%vs.77.1%,X^2=3.872,P<0.05);2组患者血压、神经功能缺损评分、ET-1、血液流变学指标水平均显著降低,SOD活性均明显升高(P<0.01),其中观察组改善程度明显优于对照组(P<0.01);2组患者治疗期间均未出现明显不良反应。结论依那普利治疗老年脑血栓患者疗效确切,可显著降低ET-1,提高SOD水平,降低血液黏稠度,安全可靠,值得临床推广应用。
Objective To observe the clinical effect of enalapril in treatment of senile cerebral thrombosis and its influence on the level of endothelin 1（ET-1）,superoxide dismutase（SOD）.Methods Selected 96 cases of elderly patients with cerebral thrombosis in the Department of internal medicine of Affiliated Hospital of Jining Medical College from October2014 to October 2015 as the research object,according to the random number table method,they were divided into observation group with 48 cases and control group with 48 cases.Patients in both of the 2 groups were received the conventional treatment of control of blood glucose,blood lipid and blood pressure;control group given dextran 500 ml intravenous drip,1 times / d;Danshenchuanxiongqin injection 10 ml solution in 0.9%NaCl solution of 250 ml by intravenous drip,1 times / d;at the same time oral nicotinic acid 50 mg,3 times / d.The observation group also added compound enalapril tablets 5 mg,2 /d.The 2groups of patients received 1 week as 1 courses of treatment,2 weeks of continuous treatment.The clinical effects were compared between the 2 groups,blood pressure,nerve function defect score,ET-1,SOD content and blood rheology changes were observed.Results After the treatment,the total efficiency in observation group was higher than that of the control group,the difference is statistically significant（91.7%vs.77.1%,X-2=3.872,P〈0.05）;level of two groups of patients＇ blood pressure,neurological deficit score,ET-1.blood rheology index were significantly decreased,the activity ol SOD were significantly increased（P〈0.01）,which the observation group unproved significantly superiort to the control group（P〈0.01）;during the treatment,2 groups of patients did not appear obvious adverse reactions.Conclusion Enalapril in the treatment of senile cerebral thrombosis is curative effect,it can markedly decrease ET-1,increase the level ol SOD,reduce blood viscosity,it is safe and reliable,it is worth of clinical application.
