摘要
目的:比较预埋留置针与带翼头皮针在肿瘤放射治疗增强CT模拟定位的量化比较,研究预埋套管针在其中的应用及效果观察。方法:选取1912例接受放射治疗前增强CT定位的患者,按照CT定位日期将单数日定位分为对照组(965例),双数日定位分为观察组(947例)。对照组患者行增强CT扫描前技师摆位后进行头皮针穿刺,连接高压注射针筒;观察组患者增强CT模拟定位预埋留置针,于增强CT前检查回血正常后连接高压注射针筒。分析比较两组患者穿刺成功率、外渗发生率、扫描成功率、静脉炎发生率以及平均定位时间。结果:观察组穿刺成功率、扫描成功率明显高于对照组,两组比较有差异(x^2=23.701,x^2=8.874;P<0.05);外渗发生率、静脉炎发生率明显低于对照组,两组比较差异显著(x^2=4.349,x^2=5.907;P<0.05);观察组较对照组缩短了平均定位时间,两组比较有差异(t=48.592,P<0.05)。结论:预埋留置针在肿瘤放射治疗增强CT模拟定位中,极大提高了穿刺成功率和扫描成功率,降低了造影剂外渗发生率和静脉炎发生率,缩短了放射治疗患者增强CT模拟定位时间及等候时间。
Objective：Comparing the effects of enhanced CT simulation orientation in tumor radiotherapy between embedded indwelling needle method and winged scalp needle method, and the application and effects of embedded trocar.Methods： Sampled 1912 cases who enhanced CT simulation orientation cases in department of radiotherapy, the singular date was set as control group（965 cases）, and the dual was set as observation group（947 cases）. The control group got enhanced CT scan with technician position first, and then scalp needle acupuncture connected with the high pressure injector. The observation group got embedded indwelling needle and connected with high pressure injector after blood return before enhanced CT scan. Analyzing and comparing the two groups of their puncture success rate, external exudation rate, average orientation time, scan success rate and phlebitis rate.Results： The success rate of puncture and scan in the observation group are significantly higher than those in the control group. There are statistical differences between the two groups（x2=23.701,x2=8.874,P〈0.05）. Meanwhile, the external exudation rate and phlebitis rate of observation group are significantly lower than those of control group. （x2=4.349,x2=5.907,P〈0.05）. The average orientation time is reduced in the observation group（t=48.592,P〈0.05）.Conclusion：Embedding indwelling needle in enhanced CT simulation orientation of tumor radiotherapy in the observation group is significantly better than the method in the control group, as the method can relieve patients’ pain and improve work efficiency.
