摘要
目的比较并探究奥曲肽联合不同剂量垂体后叶素治疗血吸虫性肝硬化继发上消化道出血的应用研究。方法106例消化科住院并进行治疗的血吸虫性肝硬化继发性上消化道出血的患者,随机分为对照组和观察组,各53例。对照组患者使用奥曲肽0.05mg/ml进行前期静脉推注,保持25μg/h维持静脉滴注的效果,保持0.5ml/min进行维持治疗。观察组患者奥曲肽联合垂体后叶素1U/ml静脉滴注0.5h左右,保持0.5ml/min进行维持治疗。比较两组患者的临床效果、止血效果以及不良反应发生的情况。结果两组患者总有效率比较差异无统计学意义(P>0.05);两组的平均止血时间和止血量比较差异均无统计学意义(P>0.05);观察组的不良反应发生率明显小于对照组(P<0.05)。结论奥曲肽联合小剂量垂体后叶素治疗血吸虫性肝硬化继发上消化道出血不良反应较少,用药量较少,可在临床上进一步推广运用。
Objective To explore and octreotide compared different doses of vasopressin treatment applied research on schistosomiasis cirrhosis secondary to gastrointestinal bleeding.Methods 106 cases of Gastroenterology hospitalization and treatment of schistosomiasis cirrhosis with upper gastrointestinal hemorrhage patients were randomly divided into control group and observation group,53 cases in each.Control group was treated with octreotide 0.05mg/ml early intravenous injection and kept 25 g to maintain the effect of intravenous drip,kept at 0.5ml/min for maintenance treatment.The observation group with octreotide and pituitrin intravenous infusion of 1U/ml half h,0.5ml/min maintenance therapy.Compare two groups of patients with clinical effect,hemostatic effect and adverse reaction conditions.Results Two groups of patients with total efficiency differences had no statistical significance(P>0.05);two groups of average hemostatic time and bleeding amount difference had no statistical significance(P>0.05);adverse reactions in the observation group occurred rate were significantly lower than those in the control group(P<0.05).Conclusion Octreotide combined with low-dose vasopressin treatment of schistosomiasis cirrhosis secondary to upper gastrointestinal bleeding fewer adverse reactions,less dosage,can further promote the use in clinical practice.
