摘要
目的:系统评价丹红注射液联合依达拉奉治疗急性脑梗死的疗效和安全性。方法:通过检索中国生物医学文献数据库、中国期刊全文数据库、维普期刊数据库、万方期刊数据库、美国国立医学图书馆数据库从2011年1月至2016年12月的中英文文献,选择符合纳入及排除标准的随机对照试验,对纳入文献进行质量评价,采用Rev Man 5.3版软件进行Meta分析。结果:共纳入24个研究,2 266例患者。丹红注射液联合依达拉奉在疗效方面对比单用丹红注射液、依达拉奉及常规治疗有优势,疗效合并效应量(RR=1.22,95%CI[1.18,1.27],P<0.00001);在神经功能缺损改善方面优于丹红对照组,合并效应量(M D=-3.58,95%CI[-4.23,-2.93],P<0.00001);在日常生活能力改善方面比丹红注射液对照组及依达拉奉对照组有效,合并效应量(MD=15.62,95%CI[13.43,17.81],P<0.00001);而在安全性方面与对照组无差异,合并效应量(RR=1.46,95%CI[0.61,3.47],P=0.91)。结论:丹红注射液联合依达拉奉治疗急性脑梗死疗效较单用一种注射剂及常规治疗显著,能减轻近期致残率,且较安全,但仍需后续大样本、高质量文献支持研究。
Objective:To assess the safety and efficacy of Danhong Injection combined with Edaravone Injection for acute cerebral infarction(ACI). Methods: To search CBM, CNKI, VIP, WF, MEDLINE for randomized controlled trials(RCTs) of related articles from January 2011 to November 2016. After assessed the quality of studies included, Rev Man 5.3 software was used to analyze data. Results: Twenty-four studies, were included and 2 266 patients were involved. The results showed that the effective rate of Danhong Injection combined with Edaravone for ACI was better than that of control group with statistical significance(RR=1.22, 95%CI[1.18,1.27], P<0.00001). Meta-analysis on the improvement of neurological deficit and activities of daily living also showed a better effectiveness of experimental group than control group with statistical significance(MD=-3.58, 95%CI[-4.23,-2.93], P<0.00001);(MD=15.62, 95%CI[13.43, 17.81], P<0.00001). But the result showed no statistical significance in safety evaluation of two groups(RR=1.46, 95%CI[0.61, 3.47], P=0.91). Conclusion: Although the results support the conclusion that Danhong Injection with Edaravone is safe and superior to either Danhong Injection or edaravone or conventional treatment, further large sample and high quality randomized controlled trials should be carried out.
