摘要
目的:探讨小剂量糖皮质激素对急性胰腺炎大鼠高迁移率族蛋白-1(HMGB-1)、髓样细胞触发受体-1(TREM-1)、肺组织髓过氧化物酶(MPO)、支气管肺泡灌洗液(BALF)的影响,为小剂量糖皮质激素临床治疗胰腺炎提供理论依据。方法:将72只SD大鼠随机分为3组,对照组(C组)、胰腺炎组(AP组)和胰腺炎+甲基强的松龙(APM组),5%牛磺胆酸钠逆行注入胰胆管建立大鼠急性胰腺炎模型,术后3、6、12、24h分别取样测定大鼠HMGB-1、TERM-1水平及MPO活性、BALF蛋白含量,并统计各组大鼠死亡率。结果:APM组与AP组相比,术后3、6、12、24hTREM-1mRNA水平较低(t=2.787、16.241、16.892、17.239,P均<0.05);与AP组相比,术后12、24hAPM组HMGB-1水平差异有统计学意义(t=12.231、15.125,P<0.01);与AP组相比,术后3、6、12、24hAPM组MPO活性均显著降低(t=2.148、16.103、16.476、17.219,P均<0.05);APM组BALF蛋白含量较AP组明显降低(P<0.01);APM组大鼠生存率较AP明显升高(χ2=7.271,P<0.01)。结论:小剂量的糖皮质激素能够降低与胰腺炎病情相关的TREM-1、HMGB-1、BALF、MPO等因子水平,有助于改善急性胰腺炎临床治疗效果,提高生存率,值得推广应用。
Objective:To evaluate the effect of small doses of glucocorticoids on HMGB-1,TERM-1,MPO and BALF in rats with acute pancreatitis,and provide theoretical basis for the clinical small doses of glucocorticoids treatment of pancreatitis.Methods:A total of 72 SD rats in the study were randomly divided into the control group(C group),pancreatitis group(AP group)and pancreatitis+ methylprednisolone(APM Group),and 5% sodium taurocholate was retrograde injected into pancreaticobiliary duct to establish rat models with acute pancreatitis.A total of 3,6,12 and 24hafter operation,samples were collected respectively,HMGB-1levels,TERM-1levels,myeloperoxidase(MPO)activity and the protein contents of bronchoalveolar lavage fluids(BALF)were detected and death rates of all groups was counted.Results:mRNA levels of TREM-1of APM group were significantly lower than those of AP group,and the t-test results of 3,6,12 and 24hafter treatment were respectively(t=2.787,16.241,16.892,17.239,P〈0.05 for all);compared with the AP group,HMGB-1levels of APM group at 12 and 24hwere significantly lower(t=12.231,15.125respectively;P〈0.01);compared with the AP group,the MPO activity of APM group significantly decreased at all time phases of 3,6,12 and 24hafter treatment(t=2.148,16.103,16.476,and 17.219respectively;P〈0.05 for all).Compared with AP group,BALF protein contents of APM group significantly decreased(P〈0.01);compared with AP group,the survival rate of rats in APM group significantly increased(χ2=7.271,P〈0.01).Conclusions:Small doses of glucocorticoids can reduce the factors associated with pancreatitis such as TREM-1,HMGB-1,BALF and MPO,it is good for improving clinical therapeutic efficacy and survival rate of AP,and it is worth popularization and application.
