摘要
目的探讨细胞角蛋白7(cytokeratin 7,CK7)和核抗原ki-67在肺癌中的定位表达与临床病理特征和预后的关系。方法收集临床手术切除的肺癌组织蜡块62例,应用免疫组化检测组织中CK7和Ki-67的表达,结合临床病理资料和随访资料进行回顾性研究。结果 CK7和Ki-67的表达在年龄和性别的差异无统计学意义(P>0.05)。Ki-67在不同病理类型的肿瘤组织中表达差异无统计学意义(P>0.05),但CK7在不同病理类型肿瘤组织中的表达差异有统计学意义(P<0.01)。在高分化肺癌患者中CK7和Ki-67表达阳性率低于低分化肺癌患者,差异有统计学意义(P<0.01)。CK7和Ki-67在有淋巴结转移的肺癌患者中的表达阳性率高于无淋巴结转移组,差异有统计学意义(P<0.01)。CK7表达水平在病理分期较早组别(Ⅰ+Ⅱ)肺癌低于分期较晚组别(Ⅲ+Ⅳ),差异有统计学意义(P<0.01),但Ki-67的表达在两组中差异没有统计学意义(P=0.18)。CK7表达与Ki-67表达无相关性(P=0.49)。结论 CK7及Ki-67高表达与肺癌的发生、发展及转移关系密切,对判断肺癌恶性程度及预后有重要参考价值。
Objective To investigate the relationship between the expression of CK7 and Ki-67 in lung cancer and its relationship with clinical pathological features and prognosis. Methods 62 cases of lung cancer tissues were collected from clinical surgery and the expression of Ki-67 and CK7 were detected by immunohistochemistry. The clinical data and follow-up data were retrospectively analyzed. Results The positive rate of CK7 and Ki-67 in different age groups There was no statistically significant difference(P〈0.05). There was no significant difference in the expression of Ki-67 in tumor tissues with different pathological types(P〈0.05),but there was significant difference in the expression of CK7 in tumor tissues with different pathological types(P〈0.01). The positive rate of Ki-67 and CK7 in patients with high differentiation was significantly lower than that in patients with low differentiation,the difference was statistically significant(P〈0.01). The expression level of CK7 and Ki-67 in lymph node metastasis group was significantly higher than that in non lymph node metastasis group,the difference was statistically significant(P〈0.01). CK7 expression level in lung cancer pathological staging(I + II) was significantly lower than that in late stage(III + IV),the difference was statistically significant(P〈0.01),but the expression of Ki-67 in the two groups was not statistically significant(P=0.18). There was no correlation between CK7 expression and Ki-67 expression(P=0.49). Conclusion CK7 and Ki-67 are important factors affecting the prognosis of patients. The abnormal expression of the two is closely related to the occurrence,development and metastasis of lung cancer. It is important to judge the malignant degree and prognosis of lung cancer.
