缺氧诱导因子-1α、血管内皮生长因子水平对糖尿病肾脏病患者的临床诊断价值 被引量:3

摘要 目的探讨糖尿病肾脏病患者体内缺氧诱导因子-1α、血管内皮生长因子水平的变化及临床意义。方法将我院92例糖尿病肾脏病患者依据e GFR水平分为DN 1组57例和DN 2组35例作为观察组,筛选出体检健康员30例作为对照组。本文检测患者24 h尿微量白蛋白采用免疫透射比浊法,测定UAE。采用双抗体夹心ELISA法检测缺氧诱导因子-1α、血管内皮生长因子水平变化与正常对照组作比较。结果与对照组相比,DN 1组与DN 2组UAE、缺氧诱导因子-1α、血管内皮生长因子水平均增高(P<0.05)。而DKD 2组与DKD 1组UAE水平对比,差异不具有统计学意义,但缺氧诱导因子-1α、血管内皮生长因子水平高于DKD 1组(P<0.05)。并且缺氧诱导因子-1α、血管内皮生长因子与e GFR呈负相关。结论缺氧诱导因子-1α、血管内皮生长因子水平与糖尿病肾脏病严重程度有关,参与糖尿病肾病的发生及发展过程,可作为早期治疗糖尿病肾脏病的指标,为病情发生及发展程度提供重要的临床依据。 Objective To investigate the clinical value of the changes of HIF-1and VEGF levels after treatment inpatients with diabetic nephropathy.Methods Selected 92 patients with type 2 diabetic nephropathy disease were divided into two groups,one group as experimental group DN1group and DN2 group,according to e GFR result.the control group is 30 cases with healthy physical examination.The immunoprecipitation and transmission turbidity method was used to detected 24 h urine albuminand calculate urinary albumin excretion rate.The serum VEGF and HIF-1α levels were measured by ELISA.Results The control group serum levels of UAE,HIF-1α and VEGF were higher in DN1 and DN2 groups.Between DN1 and DN2 group,there was no significant difference in UAE levels(P〈0.05).levels of HIF-1α and VEGF were significantly higher in DN2 group than those of DN1 group(P〈0.05).There were negative correlation between VEGF and HIF-1α levels with e GFR level.Conclusion Levels of VEGF and HIF-1α change with the severity of diabetic nephropathy,Plasma HIF-1αand VEGF levels inpatients with diabetic nephropathy were closely related to the severity of the disease process,which is an predictor for the progression of diabetic disease,they are useful in the evaluation of development of DN.
