摘要
目的分析甲状腺疾病住院患者和住院费用的构成及影响因素,为探讨降低甲状腺疾病医疗费用提供依据。方法筛选某院2010年住院治疗的甲状腺疾病共458例,对目标人群及其住院费用进行研究,对疾病及住院患者的基本情况用构成比描述,对住院费用情况进行描述性分析,统计学方法采用t检验或方差分析。结果甲状腺炎住院患者居多,占36.5%,甲状腺肿次之,占22.9%。女性患者高于男性,占全部病例的82.5%,并多集中在中年患者。参加医疗保险(医保)的患者多于自费患者,占71.6%。住院费用构成比顺位为:西药费、材料费、手术费、检验费,中位住院费用分别为2455.4元、1434.3元、900.0元、651.0元。女性与年龄偏高的患者平均住院费用较高。医保患者费用高于非医保患者。结论甲状腺疾病重点防治人群为中年女性,不同的甲状腺病种、年龄、性别,不同医疗保障、住院时间均会使甲状腺疾病住院费用产生很大变化。
Objective To analyze the composition and the factors of inpatients and hospitalization expenses of thyroid disease, and provide a basis for reducing medical costs of thyroid disease. Methods Search the inpatients of thyroid disease in one third-level grade-A hospital of Harbin between January 1, 2010 and December 31, 2010,and analyze the target inpatients and their hospitalization expenses. Results The proportion of inpatients of thyroiditis is 36.5% which is highest and the proportion of inpatients of goiter is 22.9% which is second. Female inpatients are more than male which proportion is 82.5% and mostly concentrated in middle-aged people.Medical insurant inpatients are more than self-supporting, the proportion is 71.6%. The top four of hospitalization expenses are drug costs, material costs, operation fees, inspection fees, and the mean of hospitalization expenses are 2455.4RMB,1434.3RMB,900.0RMB,and 651.0RMB in turns.The average hospitalization expenses of female and older inpatients are higher and the hospitalization expenses of medical insurant inpatients are more than self-supporting. Conclusion Middle-aged women should be one of the emphases on the prevention for thyroid disease. The hospitalization expenses of thyroid diseases differ because of different diseases, different ages, gender,and different health care, different hospital days.
